The Case Manager Lead provides consultation, technical guidance, and quality oversight to direct service staff delivering Health-Related Social Needs (HRSN) services. This role supports case planning, workflow coordination, case review, and program compliance efforts to ensure high-quality, trauma-informed service delivery.


The Case Manager Lead collaborates closely with supervisors and multidisciplinary teams to strengthen staff performance, promote adherence to agency standards, and support effective client outcomes.


Essential Duties

  • Provides coaching, consultation, and technical assistance to HRSN case management staff.
  • Reviews case documentation and service delivery practices to ensure quality, compliance, and adherence to agency standards.
  • Supports operational coordination, workflow monitoring, and program audit activities.


Job Functions

  • Serves as a subject matter resource to HRSN staff regarding policies, procedures, statutes, and regulatory requirements.
  • Interprets and communicates updates to program rules, eligibility requirements, and agency standards.
  • Provides training and ongoing coaching related to case planning, safety planning (including domestic violence considerations), and client engagement strategies.
  • Observes staff performance and provides constructive feedback to supervisors to support professional development.
  • Conducts case reviews to assess the quality of case plans, safety plans, assessments, and engagement strategies.
  • Audits case files and documentation to ensure completeness, accuracy, and compliance with federal, state, and agency standards.
  • Identifies workflow gaps or implementation concerns and recommends corrective actions.
  • Facilitates and participates in case staffing and multidisciplinary team meetings.
  • Advises staff on caseload management, service coordination, and appropriate use of rent and utility assistance.
  • Tracks workflow and monitors deadlines to ensure compliance with program requirements.
  • Provides modeling and coaching by meeting with clients alongside caseworkers when appropriate.
  • Liaises with community partners, service providers, landlords, courts, and Coordinated Care Organizations (CCOs) to support coordinated service delivery.
  • Supports court-related processes and provides guidance to staff accompanying clients to hearings.
  • Maintains professional communication with law enforcement, attorneys, and community agencies as needed.
  • Ensures confidentiality and adherence to agency policies, trauma-informed care principles, and harm reduction practices.
  • Performs additional duties as assigned in support of program operations.


Supervisory Responsibilities

  • This position does not have formal hiring or termination authority.
  • Provides coaching, technical guidance, case review, and performance feedback to HRSN case management staff in coordination with the Program Supervisor or Program Manager.


Qualifications

  • High school diploma or equivalent required.
  • Minimum of 1–3 years of experience in social services, housing navigation, or case management.
  • Demonstrated ability to interpret and apply federal, state, and local program requirements.
  • Strong organizational, communication, and problem-solving skills.
  • Experience maintaining confidential client records and using data management systems.
  • Basic computer proficiency (Google Suite, case management software).
  • Ability to collaborate with multidisciplinary teams and community partners.
  • Valid driver’s license and reliable transportation required.


Preferred Qualifications

  • Four (4) or more years of experience in case management, HRSN, or care coordination.
  • Prior leadership or mentoring experience supporting case management staff.
  • Familiarity with Oregon Health Plan, CCOs, and OHA 1115 Waiver HRSN programs.
  • Knowledge of trauma-informed care, motivational interviewing, and person-centered planning.
  • Experience working with diverse, marginalized, or rural populations.
  • Bilingual proficiency preferred.
  • Experience using systems such as Unite Us, Epic, or Salesforce Health Cloud.
