This is a frontline staff position responsible for the holistic support of Production Assistants’ success, as well as supporting the overall success of the Employment Training (“ET”) program at Growing Home, Inc. The Case Management Coordinator reports to the Director of Employment Training. Â
RESPONSIBILITIESÂ
Case Management and Supportive Service Provision:Â
- Oversees supportive service provision for GH “clients” (both current Production Assistants and program Graduates); identifies and supports needs for ancillary referrals for health, education, counseling, housing, etc.Â
- Coordinates and participates in 1:1 counseling meetings with Production Assistants to support the achievement of participant-driven goalsÂ
- Develops and maintains referral partnerships, and works closely with the Director of Employment Training in facilitating organizational collaborationsÂ
- Provides feedback and advice to other frontline caseworkers, including support to colleagues without social work backgrounds, as well as program graduate mentorsÂ
- Serves as the lead in case noting, including providing support to other GH staff Collaborates with ET Manager to provide pre-employment and retention counseling services for PAs Â
- Participates in weekly case management meetings with the ET team and  site managersÂ
Reporting and Data Management:Â
- Maintains client files, gathers and processes necessary data for case management Â
- Provides relevant data and information for reporting purposes, maintains clear data for Data analysis, and writes reports, case studies, and assessments
Recruitment and Enrollment:Â
- Assists in recruitment by leading information and application sessions
- Collaborate with ET team for participant review, interviews, and PA hiring decisionsÂ
Curriculum Support:Â
- Assists in job readiness training, including assisting in classroom facilitation, and curriculum development/revision as neededÂ
Graduate Engagement:Â
In addition to ongoing case management and support service provision to GH graduates:Â Â
- Assists in Graduate retention and outreach Â
- Assists in Graduate engagement events, Graduate Advisory Council meetings, and other Graduate related activities as needed Â
Program Assistance:Â
- Assist in office support tasks and operations as neededÂ
- Collaborate with ET team for PA application review, interviews, and hiring decisionÂ
- Assists with PA resume creation, cover letter drafting, interview preparation, and individual job search as neededÂ
- Assists in the coordination of Employment Training events Â
- Assists with Board liaising as neededÂ
- Assists with employer cultivation as neededÂ
- Performs other duties as assignedÂ
Other:Â
- As needed, assist with Growing Home open houses and events, which may include night or weekend hoursÂ
- Assist in the collection of appropriate pictures, videos, and stories for social mediaÂ
SKILLS AND QUALIFICATIONSÂ
- A demonstrated commitment to social justice with prior experience working with low-income populationsÂ
- Comfortable working directly with marginalized populations; ability to engage with individuals in an authentic and non-judgmental mannerÂ
- Familiarity with Chicago social service resourcesÂ
- Strong written and verbal communication skills; ability to engage, inspire and influence a wide variety of stakeholders
- Ability to work closely in a team of individuals with mixed professional and educational backgroundsÂ
- MSW required; will consider those currently pursuing an MSWÂ
- Some travel for meetings will be required.Â
- Must have an aptitude and willingness to navigate public transportation throughout the greater Chicago area, or a valid driver’s license
