About this role

Role and ResponsibilitiesĀ 

TheĀ Clinical PharmacistĀ willĀ help our communitiesĀ thrive byĀ optimizingĀ medication management to allow our physicians more time to focus on patient care andĀ support care coordination for assigned patient cohorts.Ā Ā TheĀ Clinical PharmacistĀ will work closely with the care coordination team and with physiciansĀ to ensure assigned patients are on intended prescribed therapies, understand their medication therapy and receiveĀ appropriate adherenceĀ coaching.Ā

Core FunctionsĀ 

  • TheĀ Clinical PharmacistĀ reports toĀ the management staff.Ā Daily tasks will also be directed by the pharmacy services team.Ā 
  • Provides telephonic follow up with membersĀ to provide medication reconciliation and medication management to assigned patients including transitional care and chronicĀ disease support.Ā Ā 
  • Complete assigned tasks related to medication management services.Ā 
  • Working withĀ member,Ā physician,Ā healthĀ system(s), pharmacy services, andĀ otherĀ care providers to meetĀ theĀ member’sĀ individual needs based on protocols and referrals,Ā Ā 
  • Coordinating transitional management including:Ā 
  • Obtaining medication ordersĀ 
  • Complete medication reconciliation.Ā 
  • Work withĀ EngageĀ teamĀ to ensure medicationĀ providedĀ until PCPĀ follow-up.Ā Ā 
  • The Clinical Pharmacist, Engage:Ā 
  • Provides medication management servicesĀ utilizingĀ prescribedĀ workflows.Ā Ā 
  • Complete assigned appointmentsĀ in accordance withĀ workflow.Ā 
  • Assess patient adherenceĀ behaviors.Ā 
  • EliminateĀ barriers toĀ adherence.Ā 
  • AssessĀ appropriatenessĀ ofĀ therapyĀ provided by theĀ pharmacy.Ā 
  • Document appropriately inĀ Engage application andĀ EnterpriseRxĀ as applicable.Ā 
  • Work with centralĀ or localĀ pharmacyĀ toĀ obtain and processĀ prescriptions.Ā 
  • Communicate with providersĀ regardingĀ servicesĀ provided.Ā Ā 
  • Support the member education goalsĀ regardingĀ their specific chronic condition based on the Plan of Care.Ā 
  • Support the evolution of pharmacy services delivery for key patient populations if applicable.Ā 
  • All communications will be conducted in a professional manner.Ā Ā 
  • RemainsĀ objectiveĀ and unbiased decisionsĀ in theirĀ medication managementĀ reviewĀ and recommendationsĀ andĀ notĀ participateĀ in any way in theĀ delivery of care to the patient.Ā 
  • Administers tasks as detailed in the member specific Plan of Care.Ā 
  • Completes clear and concise documentation in Care Management programs.Ā 
  • CompliesĀ withĀ Customer Service expectations as applicable to the Department and Health Plan.Ā 
  • Comply withĀ all departmental policies and procedures.Ā 
  • Participate in departmental and company in-services asĀ appropriate.Ā 
  • Recognizes and adheres to the scope / limitations of job requirements.Ā 
  • Performs other related duties as assigned.Ā 

QualificationsĀ and Education RequirementsĀ 

  • PharmDĀ Ā 
  • TX state pharmacy license and/or assigned marketĀ license.Ā Ā 
  • Residential state licensed in good standing
  • Clinically required licenses in good standing
  • PIC required licenses in good standing
  • Strong patient engagement skillsĀ includingĀ active listening, de-escalation interventionĀ trainingĀ and ability to teach complex topics in an understandable manner.Ā 
  • Strong organizational and time management skills, able to work with little supervision while handlingĀ numerousĀ projects at once.Ā 
  • Excellent written/verbal communication skills, especially the ability to communicate telephonically in a professional and effective manner.Ā 
  • Computer literacy in Word, Excel.Ā
