Pay: $105,000-$110,000 per year plus up to $32,000 in yearly bonuses

We have created an opportunity for home health branch clinical leadership to be free from high-volume, process-intensive back-office functions with centralized support. This allows our Clinical Managers to have full focus where it matters most: active clinical oversight, enhanced skills training and development, and driving systems and behaviors that keep patients safe and out of the hospital. Our technology also eliminates administrative burden so that clinicians can focus on what they were trained to do: deliver excellent patient care.

Position Type: Onsite, branch-based leadership role. This is an active, high-intensity clinical oversight position โ€” not a desk role.

Position Overview

We are looking for a Clinical Manager who can lead clinical execution with high standards and high intensity. This is not a passive "clinical support" role. The branch Clinical Manager owns day-to-day clinical quality, staff development, and rehospitalization prevention for their branch. You are accountable for outcomes - not just activities. You will partner closely with operations, scheduling, and leadership to keep care delivery on track, and you will be a key leader in how we scale safely.

What You'll Do

Own Clinical Execution for Your Branch

  • Provide direct clinical guidance to clinicians as needed

  • Respond to clinical calls (patients, caregivers, providers) and identify actions needed with appropriate clinical judgement

  • Monitor daily on-call events to identify urgent patient status changes

  • Conduct real-time evaluation reports from assessing clinicians to identify immediate patient needs, ensure care plan is complete, provide guidance for visit utilization, and follow up on verbal plan of care approval to ensure physician orders are timely and complete

  • Complete transfer reviews daily to assess hospitalized patients including root cause analysis and action planning as needed for areas of improvement

  • Review high-risk patients and coordinate proactive intervention to prevent avoidable events

Lead and Develop Field Clinicians

  • Coach clinicians on clinical reasoning, visit planning, patient education, and documentation quality

  • Set and reinforce expectations for reliability, professionalism, and patient experience

  • Address performance issues quickly and fairly, including structured improvement plans when needed

  • Facilitate new clinician orientation: system orientation, SOP review, company orientation, and skills training

  • Lead and oversee skills validations to ensure clinical competency before independent deployment

  • Provide oversight of Field Supervisors โ€” match, guide, and evaluate pairings for new hires and Field Supervisory

Oversee Care Coordination and Care Planning

  • Prepare for and lead weekly case conference: review patient progress and build actionable discussion agendas for care planning decisions

  • Serve as a calm, decisive escalation point for clinical questions and patient risk

  • Know when to intervene directly and when to empower field staff with guidance

  • Coordinate with leadership on high-risk cases, incidents, and sensitive patient situations

Lead Quality & Compliance

  • Review Quality Assurance review issues and facilitate direct communication and education with the responsible clinicians as needed to improve documentation accuracy / collaboration with Quality Assurance team that is responsible for OASIS review and coding

  • Track HHCAHPS results and quality KPIs; identify trends and implement coaching or process changes

  • Maintain branch survey readiness โ€” own documentation quality, clinical compliance, and field staff preparedness

  • Perform field supervisory visits to ensure adherence to infection control, safety, and compliance; assess and train on clinical skills, and improve documentation quality

Be the Escalation Leader

  • Make calm, decisive escalation decisions for high-risk patients, safety concerns, and incidents

  • Coordinate with leadership on sensitive patient situations, hospitalizations, and quality investigations

Must-Haves

  • Active RN license (required)

  • Home health experience

  • Ability to coach, mentor, and train clinical staff across skill levels

  • Strong clinical judgment and a structured approach to prioritization and escalation

  • A strong sense of personal accountability โ€” you take ownership of clinical outcomes through your clinical staff

  • Ability to communicate clearly across clinical and operational teams

  • Willingness and ability to be present onsite at the branch as an active clinical leader

Nice-to-Haves

  • Experience partnering closely with operations teams to prevent service failures

  • Experience leading new hire clinical orientation and skills validation programs
