Reports to: Sr Case Coordinator

Hourly Rate: $20.08-$22.97

 

Bancroft is looking for a Case Manager/Coordinator. The Case Manager/Coordinator works collaboratively with Bancroft program leadership, clinical staff, Finance staff, with guardians/family members and external case managers/funders to ensure that individuals served, and  individuals accepted for admission, meet and maintain funding eligibility for Bancroft services.

Responsibilities

Bancroft Neuro Rehab Case Managers are responsible for:

  • Manage aspects related to assigned caseload that pertain to access to Bancroft services, family advocacy, and funding and ensure cases are approached in a thoughtful, holistic manner that reflects Bancroft’s core values. 
  • Schedule and coordinate the annual plan meetings and ensure that all care team members are included. 
  • Ensure that all documentation for the annual plan is promptly prepared and provided to the appropriate parties. 
  • Ensures that the annual plan accurately captures needed services and ensures that funding authorizations are in place for the services.
  • As needed facilitates eligibility and benefit checks and oversees the process of redetermining and/or restoring eligibility for benefits and entitlements. 
  • Assist with securing funding authorizations for durable medical equipment and adaptive devices and assist resolving any medical insurance issues.
  • Support the process when individuals transfer services to a new program or service line by ensuring that transfer meetings are held with external care team members’ input and that all documentation is in place for no disruption in funding.
  • For new admissions, attend the pre-admission and admissions meetings to meet the external care team members and to learn about the service needs of the individual being admitted and coordinate the discharge planning processes.
  • Collaborate with care team members in developing, authoring and overseeing the Annual Plan for assigned caseload and distribute and/or electronically share with internal and external care team members. 
  • Generate monthly treatment plan reviews with internal and external care team members.
  • During IDT/Annual plan meetings provide and obtain information regarding the Plan, review baseline goals, identify individual strengths and weaknesses, evaluate progress of persons served regarding attainment of goals, determine the need for modifications to existing goals, combine goal teaching or rehabilitation and development strategies for best possible outcome, and discuss concerns regarding specific persons served to ensure overall Plan effectiveness. May coordinate meeting logistics.
  • Collaborate with external case managers to secure new, appropriate care outside of Bancroft for individuals being discharged. 
  • Schedules therapies for newly admitted individuals
  • Communicate with billing and finances any changes in services and insurances.
  • Utilize the re-authorization process for securing authorization for MLTSS.
  • Ensure outcome assessments are completed and entered into CCP.
  • Ensure care plan for services is up to date and Services are provided based on Care Plans.
  • In collaboration with either the IDT or TDT, develops data collection systems to monitor established goals for persons served, determine need for improvements or modifications, adjustment of goal criteria, etc.

Qualifications

Education & Experience:

A bachelor’s degree is required. Candidates must meet one of the following qualifications:

  • BSW or MSW

  • Bachelor’s degree in a health services– or human services–related field
  • Certification as a Rehabilitation Counselor (CRC)

  • Certification as an Insurance Rehabilitation Specialist (CIRC)

  • Certification as a Brain Injury Specialist (CBIS)

Special Skills:

Proven ability to effectively communicate both verbally and in written form required.  Effective and proven ability to prioritize, multi-task; management skills required, as well as flexibility and adaptability. Ability to assess situations, problem solve and delegate tasks. Effective interpersonal and organizational skills. Basic math skills required. Operative knowledge of MS Office Word and Excel spreadsheets preferred.  Valid driver’s license required in incumbent’s legal name and current address with no provisional restrictions
