What You'll Be Doing

A Cancer Patient Navigator is needed to support a population health program at Hudson Regional Health in Hudson County. Primary duties are to assist patients who identified as a high risk for developing lung and or colo-rectal cancer and overdue for cancer screening.  The Navigator plays a pivotal role guiding the most at-risk patients, helping people prevent cancer, screen for cancer, and link patients to quality cancer treatment.

 

Under the supervision of the project director and system director cancer services, the Navigator provides individualized assistance to patients who are due for Lung/Colo-rectal cancer screening. Often the first point of patient contact; this position ensures that eligible patients receive lung and colorectal cancer screening, and follow-up with linkage to care.  The Navigator needs to be self-motivated, driven by improving health outcomes and health equity, and with a solid ability to operate autonomously with guidance.

 

Primary Responsibilities (include but are not limited to): 

  • Performs outreach to Primary Care Physicians and their patients that are identified eligible for cancer screening.
  • Facilitates and assists scheduling patients’ appointments for low dose CAT scan of the lung and colonoscopy, FIT test, or Cologuard testing for colorectal cancer screening.
  • Makes confirmation calls/mailings/emails to patients regarding future appointment dates.
  • Educates patients on care processes.
  • Develops relationships with health care providers within and outside the health center/practice.
  • Supports Hudson Regional Health quality improvement efforts regarding cancer screening and referrals.
  • Identifies patient barriers to care such as transportation, insurance, and language barriers, and refers patients as needed to additional resources to support engagement in care.
  • Assists in helping patients arrive at scheduled appointments on time and assist coordinating transportation when needed.
  • Using Electronic Health Record (EHR) system, enter patient encounter data and follow-up schedule. Additionally, tracks navigation services, records messages from patients, and contributes to clinic tracking workflows.
  • Tracks appropriate patient-level quality outcomes for program monitoring and evaluation.
  • Attends training and learning opportunities that expand content knowledge and best practices.
  • Adheres to clinic departmental policies and procedures, which include Joint Commission (JC) standards, Patient Safety initiatives, Patient Rights, and Health Insurance Portability and Accountability Act (HIPAA) Privacy standards.
  • Understands and adheres to CarePoint’s compliance standards as they appear in Hudson Regional Health’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
  • Performs Other related duties as assigned.
  • What We're Looking For

    Education, Skills and Experience Requirements:

    • Prefer Medical Assistant plus two (2) years of experience with health care clinic operations in a patient-facing role
    • Demonstrated interest in and knowledge of community health care and primary care.
    • Electronic health record (EHR) experience required
    • 2-3 Years Medical Office experience preferred.
    • Proficiency with Microsoft Word, Excel, and databases required
    • Ability to work collaboratively with physicians, staff, and external organization.
    • Strong communication skills with diverse populations required. Solid relationship- building and interpersonal skills. Excellent time management skills. Excellent coordination skills, including multitasking and setting priorities on work assignments.
    • Knowledge of medical terminology and organization services.
    • Outstanding verbal and written communication skills, customer service skills, phone etiquette and works well in a fast-paced dynamic atmosphere. Team-oriented mentality.
    • Ability to multi-task and work independently while being detail oriented.
    • High degree of independence, flexibility, initiative, and commitment. Demonstrated awareness and value of cultural competence.
    • Displays extraordinary customer service skills in person, telephone, and written communications.
    • Bilingual (English/Spanish) speaking and writing ability preferred.
