Immanuel Pathways PACE® is seeking a Nurse Enrollment Coordinator to work at 1702 North 16th Street, Council Bluffs, IA 51501. You will work with our Immanuel Pathways Day Center and at participant's personal homes. We reimburse $0.725/mile for travel.

 

The schedule for this position is Monday through Friday 8:30 am to 4:30 pm. 

 

Pay will be based upon experience, certifications, skills and education.

Job Duties & Skills Required

What you will do:

The Nurse Enrollment Coordinator plays a key role in welcoming new participants into the PACE (Program of All-Inclusive Care for the Elderly) program. This position combines clinical expertise with strong communication and organizational skills to guide potential participants and their families through the enrollment process. The Nurse Enrollment Coordinator conducts initial in-home clinical assessments, completes all required enrollment documentation, and serves as the primary liaison with the State to ensure timely and accurate processing of applications. Supports and lives out Immanuel’s Mission and CHRIST Promises.

Key Responsibilities and Duties of the Job

Clinical Assessment

  • Conduct comprehensive initial nursing assessments for all prospective PACE participants, including functional, medical, psychosocial, and safety evaluations.
  • Determine preliminary clinical eligibility in alignment with PACE and state-level criteria.
  • Collaborate with the interdisciplinary team (IDT) to review assessment findings and support enrollment decisions.

Enrollment Coordination

 

  • Provides information about the program of All-inclusive Care for the Elderly (PACE).
  • Takes incoming calls and provides information about the Immanuel Pathways Program.
  • Speaks to potential PACE participants in their home, hospitals or other settings as part of the communication process.
  • Provides tours of the Immanuel Pathways Center.
  • Guides and assists potential participants through the enrollment process.
  • Answers all medical and financial questions potential participants/family have regarding the Pathways PACE program.
  • Collects financial and medical information for all potential enrollees.
  • Manages the full enrollment workflow from referral through program start date.
  • Completes all required PACE enrollment paperwork, consent forms, and documentation with accuracy and attention to regulatory requirements.
  • Maintains organized, compliant records in accordance with PACE, CMS, and State guidelines.
  • Keeps potential participants/family updated on enrollment process.
  • Inform potential participants/family of qualification or non-qualification for PACE.
  • Provides Immanuel Pathway staff information and updates throughout enrollment process.
  • Coordinates and facilitate internal Service Plan Meeting to review care needs of all potential enrollees.
  • Serves as the primary point of contact for prospective participants, families, caregivers, and referral sources throughout the enrollment process.
  • Provides clear, compassionate explanations of PACE services, eligibility requirements, and program expectations.
  • Communicates regularly with the State regarding level-of-care determinations, required documentation, and enrollment status updates.
  • Coordinates with internal departments—including social work, transportation, therapy, and primary care—to ensure a smooth transition into the program.
  • Provides alternative community resources to applicants who do not qualify for Immanuel Pathways (PACE).

 

Compliance & Reporting

  • Completes Security Review process for systems access in the respective state in which he/she is employed.
  • Maintains up to date knowledge of system/process and communicates to IDT as applicable.
  • Maintains Customer Relationship Management database.

Other

  • Participates and supports the Quality Improvement Initiatives.
  • Practices Universal Precautions and follows appropriate infection control as applicable.
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
  • Performs other duties as assigned or requested.

 

QUALIFICATIONS

Education-

  • Current license as a Registered Nurse (RN) in the respective state in which he/she is employed required.

Experience-

  • Two (2) years of nursing experience; geriatric, community/home health, or care coordination experience required.
  • One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.
  • Equivalent years of education may substitute for experience requirement.

Other Requirements –

  • Must have medical clearance for communicable diseases and up-to-date immunizations before joining Immanuel Pathways.
  • Must have a valid driver’s license, proof of insurance and have reliable means of transportation for driving up to 100 miles daily.
  • Must have a professional
  • Basic Life Support (BLS) Skills i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aide (FA).

 

KSA- Knowledge Skills and Abilities-

  • Knowledge of physical, psychosocial, behavioral, and family needs of
  • Knowledge of the PACE philosophy and eligibility/denial
  • Knowledge of the service area and its community’s health and social service delivery systems.
  • Knowledge of the Medicare and Medicaid
  • Skilled in written and oral
  • Skilled in basic/intermediate computer knowledge and Microsoft applications.
  • Ability to chart via Electronic Health
  • Proven experience and basic computer proficiency (internet, email, Microsoft Office).
  • Ability to effectively speak with perspective participant/family and present PACE philosophy and model and to prospective participants.
