Samaritan Daytop Village is a leading human services organization in New York State serving over 38,000 New Yorkers annually across more than 60 locations through Transitional Housing, Community Based Services, and Residential and Outpatient programs. First Steps Intensive Crisis Stabilization Center (ICSC), operated by Samaritan Daytop Village in Rockland County, is a 24/7 program dually licensed by OMH and OASAS to provide rapid, short-term support for children ages 5+ and adults in mental health or substance use crisis. Our multidisciplinary team utilizes a trauma-informed, harm-reduction framework to stabilize individuals and connect them with long-term care while avoiding unnecessary emergency room visits.Â
We are seeking a Licensed Practical Nurse (LPN) to provide essential clinical support and compassionate patient care. Working under the direction of Registered Nurses, Nurse Practitioners and Physicians, you will play a key role in medical stabilization, vital sign monitoring, and the daily health management of individuals experiencing acute mental health or substance use emergencies. This role requires a dedicated professional who can balance technical nursing duties such as wound care, medication coordination, and EHR documentation. As a vital member of our 24/7 multidisciplinary team, you will ensure a safe, organized, and respectful environment for clients navigating the road to recovery.Â
Responsibilities
- Conducts patient intake, takes medical histories, and completes data entry in the clinical systemÂ
- Monitors vital signs, including blood pressure, pulse, temperature, and weightÂ
- Administers injections, performs clinical tests, and provides wound care as orderedÂ
- Follows physician, nurse practitioner, and RN orders to provide direct patient care and medication coordinationÂ
- Documents detailed reports of care and maintains accurate charting in the Electronic Health Record (EHR)Â
- Reports significant changes in a patient’s physical or mental status immediately to the RN or PhysicianÂ
- Communicates with pharmacies to place prescription orders while maintaining strict confidentialityÂ
- Reinforces clinical instructions given by the RN or Physician to ensure patient understandingÂ
- Utilizes therapeutic communication to assist patients and initiates emergency assistance as per facility policiesÂ
- Manages clinical workspace, including stocking supplies and preparing patient charts for the teamÂ
- Participates in staff development, nursing conferences, and ongoing clinical trainingÂ
- Collaborates with a multidisciplinary team to ensure safe and effective patient careÂ
- Maintains a neat work area and ensures patient rooms are fully equipped for daily operationsÂ
- Engages with individuals from diverse backgrounds to explain medical instructions with clarity and empathyÂ

Qualifications
- Active, unrestricted New York State Licensed Practical Nurse (LPN) licenseÂ
- Current BLS certificationÂ
- High School Diploma or GED and graduate of a formal Technical or Vocational LPN ProgramÂ
- Minimum of one (1) year of experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI)Â
- Available to work a flexible schedule as needed in response to participant and staff needsÂ
- Computer literacy including proficiency in Microsoft Office Suite and Electronic Health Records (EHR)Â
- Knowledge of medical terminology and ability to multi-task with excellent organization skillsÂ
- Experience working directly with people from diverse racial, ethnic, and socioeconomic backgroundsÂ
- Ability to work effectively both independently and as part of a multidisciplinary clinical teamÂ

Additional InformationÂ
This is a full-time non-exempt role with an hourly rate of $27.47-$32.06 (annualized at 35 hours a week to $50,000-$58,350) with a full benefits package. Benefits include medical, dental, and vision insurance, 25 days of paid time off, access to a retirement account with an employer to match and tuition reimbursement. Â
