Come work as a Medical Coordinator – Street Health & Supported Housing with Vancouver Coastal Health (VCH)!

Reporting to the Medical Director of Homelessness, Supported Housing and Complex Care in Vancouver Community, the Medical Coordinator, Street Health and Supported Housing provides physician leadership across four interdisciplinary teams (MoDe, ICRT, INO, IHOT) and designated supported housing sites, including Victory House. The teams are as follows:

  • Intensive Health Outreach (IHOT) team - provides low-barrier interim health care, and psychosocial support for unhoused/vulnerably housed people in Vancouver. With an ultra-low barrier clinic that runs on Mondays, Wednesdays and Thursdays at the Molson Overdose Prevention Site (across from InSite)
  • Mobile Crisis De-escalation (MoDe) team - a non-police, same-day crisis response team individuals in the inner city who are experiencing a mental health or substance use crisis
  • Indigenous Crisis Response Team (ICRT) - a non-police, mental health support/ response team for Indigenous folks across Vancouver
  • Indigenous Health Outreach Team (INO) - provides culturally-forward, relationship-centred interim health care, and psychosocial support for Indigenous folks across Vancouver.

 

This role operates within a social medicine framework, recognizing housing as a health intervention and emphasizing longitudinal, relationship-based care across outreach, and supported housing environments.

 

The Medical Coordinator works in dyad partnership with the operations manager and collaborates closely with nursing, allied health, and community partners to support integrated, equity-oriented care delivery.

Qualifications

Key accountabilities:

  • Clinical Leadership & Service Integration
    • Provide medical leadership across MoDe, ICRT, INO, and IHOT teams, ensuring high-quality, equitable, and trauma-informed care
    • Support integration of services across outreach, inreach, supported housing settings
    • Provide oversight of physician services at designated supported housing sites, including Victory House
    • Promote continuity of care across transitions (acute → community → housing) through attendance/sponsorship of complex care rounds and other case management functions
    • Act as a liaison between frontline clinicians and program medical leadership
    • Provide regular performance reviews to front-line physicians
  • Operations, Scheduling & Workforce Planning
    • Coordinate scheduling, coverage, and locum support, as well as the distribution of call schedules
    • Support recruitment, onboarding, and retention of medical staff
    • Session management - approves sessions of site(s) physicians, monitors session usage, and informs the medical leaders of any concerns related to session hours or utilization.
  • Quality Improvement & Learning Health System Development
    • Stand up our vision of an urban health learning health system through ongoing support for QI and the building of a community of practice
    • Lead and support equity-oriented quality improvement (QI) initiatives across teams
    • Embed continuous improvement methodologies (e.g., PDSA cycles) into clinical practice
    • In keeping with our vision to sustain a community of practice for our portfolio:
      • Facilitate clinician engagement in research, evaluation, and knowledge translation
      • Identify and address learning needs specific to street health (e.g., harm reduction, brain injury, consent/capacity)
      • Support interdisciplinary case-based learning and clinical rounds
      • Coordinate medical learner placements across outreach and supported housing environments
    • Critical incidents - gather information about any on-site incidents in collaboration with the multidisciplinary team and inform medical leadership if significant harm has occurred to a patient or a staff member. A member of the team should submit a PSLS report which will generate a VC quality review process

     

    The expected time commitment is approximately 0.2 FTE (8 hours/week).
