As a member of the Healthcare team, and lead by the Chief Medical Officer, the Family/Adult Nurse Practitioner is vital to the successful completion of the organization goal. Each clinical team member receives performance measures and patient satisfaction results and is given the opportunity to participate in the development of a plan for improvement through regular QA meetings and or action plan development.

The Family/Adult Nurse Practitioner is directly responsible to the Chief Medical Officer of Central Jersey Medical Center, Inc.

Responsibilities

  • Assesses the individual and family health status of adults, considering the needs as seen by the individual.
  • Secures health histories, performs physical examinations and screening tests.
  • Records acquired data in a systematic and accurate form.
  • Contributes to the health education of individuals and groups, and applies methods designed to increase each personโ€™s motivation to assume responsibility for his/her own health.
  • Plans and implements routine care practices in collaboration with medical and other health professionals.
  • Identifies and helps in the management of social, economic, and environmental influences affecting the patient and his family.
  • Assesses and manages common acute and chronic illnesses in accordance with protocols.
  • Prescribes selective medications while understanding orders of the physician.
  • Assists in managing and planning for referral of complex health problems.
  • Assists in the establishment of policies and procedures relevant to comprehensive, longitudinal care.
  • Supports and carries out such policies and procedures.
  • Contributes to the comprehensive care of sick adults in ambulatory care setting in collaboration with the responsible physician.
  • Collaborates with the primary care physician in the development and coordination of health care plans involving family and community to enhance the quality of health care and diminish both fragmentation and duplication of services.
  • Participates in outreach programs and screening programs for case identification and follow-up, and community health education.
  • Provides night call coverage by phone.
  • Within the health care team, uses principles of group development to better understand his/her colleagues and to make the health care team more efficient and effective.
  • The nurse practitioner will perform other related duties and responsibilities as assigned by the CMO or CEO.
  • Performs independently as specified by NJ Board of Medical Examiners.
  • Specialize in the maintenance of health, the prevention of disease and complications, the diagnosis and management of common acute and chronic illnesses of adults.
  • Performs other related duties as assigned or requested.

Qualifications

  • Graduate of an accredited college with a certificate of Masters of Science in Nursing degree
  • Graduate of an accredited Adult/Family Nurse Practitioner Program
  • Must possess a State of NJ License
  • Must possess a CDS License and DEA License issued by the State of NJ
  • Certified as an Adult Nurse Practitioner by an accredited association (ANCC, AANP)
  • DEA X waiver
  • Must possess a current CPR/BLS certification
  • At least 2 yearsโ€™ experience in current field is preferred; or equivalent combination of education and experience
  • Prior experience in a comprehensive, ambulatory care setting is preferred; experience in a health care team delivery; experience with inner city populations
  • Bilingual preferred but not required.
