Inspire Lancashire are recruiting a Young Adult Transition Worker !

The Young Person to Adult Transition Worker is integral role in transitioning people through their treatment from a young person’s specialist services into to adult services; working closely with the Young Person service collaborating in making the transition and change in service cultures as smooth and efficient as possible is key to ensure that we continue to meet the needs of young adults accessing support.

 

You will be responsible for a caseload of young adults affected by drugs/ and or alcohol with our Burnley Team. You will always maintain high professional standards and ensure a service user focused approach in all aspects of delivery; You will lead by example in creating a professional and person-centred team culture focused on excellent service delivery.

 

Hours: Part Time, 30h per week

Contract Type: Fixed Term until 22nd of August 2027 (Maternity Cover) 

Location: Burnley

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

 

  • Engaging and maintaining engagement with service users.
  • Supporting young adults transition between young people’s services and integrated adult services. 
  • Undertaking assessment of individual need and formulating treatment and risk management plans using evidence based assessment and screening tools linked to substance use .
  • Delivering a range of appropriate interventions such as:• Identification and Brief Advice • Extended Brief Interventions & structure Psychosocial interventions
  • Provide Harm reduction advice, information and Interventions
  • With oversight of the Service Healthcare and Medical team support young adults to access medical and clinical interventions
  • Delivering relapse prevention interventions. 
  • Working alongside families and carers relevant to the young adult’s care plan. 
  • Working alongside relevant agencies involved in the young adult’s care plan. 

 

About you:

  • Have a strong understanding of substance misuse issues and experience of working within a similar field
  • Experience of working with groups/pods and managing group dynamics, such as conflict or challenging behaviour.
  • Be an excellent communicator, both verbal and written with good IT skills
  • Proactive and able to work on own initiative with excellent time management and prioritising skills
  • Have a good working knowledge of mental health interventions, services, and good practice
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
  • Ability to manage change successfully in a way that prioritises the needs of service users
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
  • Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others

CGL points 23 to 28 (£28,557.79 - £32,802.41)
