As a Recovery Coordinator with Change Grow Live you’ll work with people who use our services, helping them get to where they want to be.

With this role you’ll be working as part of the Switch Team, based within Hull Renew

The team supports people with their or their loved ones’ drug or alcohol issues. 

You will work collaboratively with partnership agencies to support people who have been placed in accommodation (supported lettings) by the local authority.

In order to deliver coordinated and intensive targeted support to this cohort to reduce the likelihood of tenancy breakdown, a Multi-Disciplinary Team (MDT) approach will be adopted that will include input from specialist alcohol & drug provision, tenancy sustainment support and other disciplines, as appropriate to each individual’s assessed needs.

 

You will manage a diverse caseload and deliver intensive drug/alcohol support working holistically with other agencies that may be involved in supporting the person.

 

Where: Hull

Hours: Full Time, 37.5 per week 

Contract: Permanent 

 

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. 

Responsibilities

About You:

  • Work as part of multi-disciplinary team to deliver high-quality support to rough sleepers and those at risk of homelessness.
  • To arrange and attend professional meetings with services in relation to the people we support i.e. MARM meetings
  • Foster a compassionate and inclusive environment where every person is treated as an individual, with flexibility, curiosity, and active listening being central to our approach.
  • Collaborate with your team and service safeguarding experts to identify and mitigate risks for people we are supporting.
  • To co-ordinate and lead a pro-active solution focussed approach for people, ensuring they can access appropriate treatment options
  • To provide personalised assessments and develop and regularly review holistic recovery and support plans with the person you and other professionals to reduce barriers to recovery and ensuring their needs are met.

 

About You

 

  • Experience of working within housing or homelessness services.
  • Working knowledge of mental health, substance misuse and complex needs.
  • Knowledge of homelessness, housing, benefits, and welfare.
  • Knowledge of homelessness law.
  • Working towards a professional housing / homelessness qualification.
  • Working with vulnerable groups in either a professional, personal or voluntary capacity.

Salary Range (pro rata if part time)

CGL points 23 to 28 (£28,557.79 - £32,802.41)
