Overview
Shift: 8:00 AM - 4:30 PM | 3 days/week (+ every 4th weekend)

Provide compassionate care and make a meaningful difference in patients’ lives as a Home Health/Hospice Aide. In this role, you’ll deliver high-quality personal care, homemaker services, and companionship to patients in their homes while supporting the RN Case Manager to ensure coordinated, patient-centered care.Â

Responsibilities
- Assist patients with daily living activities including bathing, dressing, oral hygiene, and mobility Â
- Provide light housekeeping, meal preparation, and companionship Â
- Support medication administration and simple procedures under RN guidance Â
- Observe and report changes in patient condition or home environment to the RN Case Manager Â
- Maintain accurate documentation and ensure compliance with care plans and regulatory standards Â
- Participate in interdisciplinary team meetings, in-services, and quality improvement initiatives Â
- Build strong, compassionate relationships with patients, families, and caregivers while maintaining professional boundariesÂ
Qualifications
Education:Â Â
- Completion of a 120-hour Nursing Assistant course. Â
- High School Diploma/GED High School diploma or GEDÂ
License(s)/Certification(s):Â Â
- Valid Driver’s License (IA, IL, or WI) – Required upon hire Â
- Basic Life Support (BLS) Certification – Maintain current certification through the American Heart Association (Required upon hire) Â
- Certified Nursing Assistant (CNA) – State of Iowa (IADIA) or Illinois (IDPH) preferred Â
- Child Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire Â
- Adult Dependent Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire
Experience:Â Â Â
- Completing of a Nursing Assistant Training Program of a minimum of 75 hours with no continuous period of 24 months without compensation for Home Care or Hospice Aide/CNA work after completion Â
