Shift: Full-time; M-F 8 AM – 430 PM; 6 weekends per year; 1 holiday per yearÂ

The medical social workers job is to perform assessments with patients to assess for any physical, mental, or emotional needs. They assess the family support system and link the patient with services to help with care if needed. The social worker can also provide assistance with financial concerns such as not being able to afford medications or pay household bills. Some social workers provide counseling to patients who are suffering from depression or who are having difficulty coping. A social worker links and makes referrals to agencies to assist the patient with all of these services.

Responsibilities
- Develop and maintain the patient’s treatment plan throughout the patient’s course of treatment with input from the care teamÂ
- Keep patient family or others informed as to the patient’s progress throughout treatment as the patient desires.Â
- Complete and update safety planÂ
- Lead psychoeducational groupsÂ
- Assist in inpatient and outpatient programmingÂ
- May assist in patient roundingÂ
- Primary responsibility for coordinating care to facilitate dischargeÂ
- Make necessary referralsÂ
- Provide patient education relating to post-acute levels of care/servicesÂ
- Assure patient’s SDOH needs are met for an effective discharge planÂ
- Coordinate pharmacy and outpatient services follow upÂ
- Assure that patient has a 7 day follow up post dischargeÂ
- Align transportation and follow up with natural supports on behalf of the patientÂ
- Participate in all required trainings and educational activitiesÂ
- Participate in department level meetings and huddlesÂ
- Be responsive to email, phone messages and patient portal messagesÂ
Qualifications
Education:
- Master’s degree in Social Work requiredÂ
Experience:
- 2 years relevant social work experience in a health care setting or similar.Â
