POSITION SUMMARY:   The Community Health Worker (CHW) plays a vital role in supporting California’s Medi‑Cal transformation under CalAIM (California Advancing and Innovating Medi‑Cal) by providing relationship‑based, whole‑person support grounded in lived experience. As a trusted member of the community, the CHW builds meaningful relationships, delivers culturally responsive health education, supports care coordination, and helps individuals and families navigate health and social service systems, including CalAIM Enhanced Care Management and Community Supports. The CHW works collaboratively with the World Relief CalAIM team, healthcare providers, and community partners to reduce barriers to care, address social drivers of health, and empower clients and improve health outcomes for the communities they reflect and serve.
