THE OPPORTUNITY: MHN is seeking a dedicated and compassionate Medical Assistant to support Annual Wellness Visits (AWVs) for Medicare patients in community health center settings in the Chicago metropolitan. This role offers a unique opportunity to advance preventive, geriatric-focused care for seniors in under-resourced communities. In this position, you will help bring specialized geriatric expertise into community health centers that traditionally do not have dedicated geriatric services, improving both the level and quality of care delivered to older adults. The Medical Assistant plays a critical role in preparing patients for visits, supporting providers, and ensuring comprehensive wellness assessments that focus on prevention, early detection, and chronic condition management. This is an onsite role working directly with patients and care teams to ensure high-quality, patient-centered care for aging populations. This role will work onsite at our partner health center locations across the Chicago metropolitan including ** Chicago, Berwyn, Waukegan, Evanston, Oak Park, Melrose Park.
