Position Summary


KCHC is structured to support sustainable primary care practice. Physicians work within an interdisciplinary team and are provided with dedicated time for documentation, care coordination, and complex care planning. The model allows comprehensive care without rushed visits.

The physician works in collaboration with nurse pracitioners, social workers, dietitians, diabetic educators, nurses, practical assistance and other allied health professionals to support the health and wellbeing of patients and the community. Members of the inter-disciplinary team assist with forms, non-medical needs, and allied health support around social complexities.

Physicians join KCHC to continue practicing full-scope family medicine. Within this position, you will hold cross-appointments with Queen’s University as Adjunct Clinical Faculty and participate in teaching. This environment allows physicians to practice the full scope of comprehensive family medicine while maintaining professional sustainability.

In this role you will provide comprehensive primary health care services to individuals and families as part of an interdisciplinary team. Care includes health promotion, disease prevention, full life cycle, chronic disease management, and coordination of care with community partners.

Responsibilities

 

Client Care

  • Provide comprehensive primary care across the lifespan, including diagnosing and managing acute and chronic conditions, as well as delivering preventive care and health education.
  • Provides client centred care acknowledging the social deteminated of health in a non-judgemental, harm-reduction-oriented manner.
  • Communicates in a manner that can be understood by the client and their families and other members of the care team.
  • Shares on call duties with team of providers as applicable by ministry and group guidelines.
  • Acts as health advocate and makes effective use of health care resources (e.g. consultants, hospital facilities, e-referral and e-consult etc.) to benefit clients.
  • As a member of multi-disciplinary team, participates in case consultations, shared care with allied health professionals and specialists, and other forms of team-based collaborative care.

 

Administration

  • Contributes to developing, implementing, monitoring, and evaluating medical protocols and directives.
  • Employs a quality improvement approach to population health efforts and client care interventions.
  • Participates in team meetings, fostering a positive team environment to support effective communication and shared care.
  • Maintains accurate records of client visits and encounters, including phone encounters.
  • Supervises medical students and residents.
  • Assists in health services planning and programming by participating on committees, supporting programs, and sharing information about identified health needs within the community as obtained through client encounters.

  • Comply with all relevant legislation and KCHC policies, including privacy laws
  • Commits to acquiring an understanding of the importance of trauma responsiveness and the impact of Adverse Childhood Experiences (ACEs)

Basic Education and Experience Requirements

  • Certification in Family Medicine by the College of Family Physicians (CCFP).
  • Registration with the College of Physicians and Surgeons of Ontario is in good standing.
  • CMPA (or equivalent) malpractice insurance.

 

Knowledge, Skills and Abilities

  • Excellent oral and written communication skills (French language is an asset).
  • Experience working with diverse or underserved populations considered an asset
  • Experience with Quality Improvement (asset).
  • Demonstrated ability to attend work on a regular basis and ability to meet the physical demands of the position.
  • Excellent organizational skills and above average attention to detail.
  • Excellent interpersonal skills with ability to engage marginalized clients, and a commitment to being a team player.
  • Demonstrated ability to work respectfully with Indigenous communities.
  • Proficiency in the use of computers and relevant software applications (Microsoft Office), including Electronic Medical Record software (PS Suite preferred).

  • Current and satisfactory Criminal and Vulnerable Persons Check.
