The Cohen Clinic network in Asia offers a unique opportunity to serve U.S. military families stationed at five U.S. military bases in the region. Point Clinical Services PLLC provides the licensed clinicians that deliver clinical services at the Cohen Clinics. If selected for this position, relocation assistance will be provided to move you to South Korea to deliver clinical services at the Cohen clinic as applicable.
What you’ll do
We are seeking to add an Intake Coordinator to our team. Under the direction of the Clinic Director, you are responsible for providing support to the Cohen Clinic as the first contact for services. You will conduct phone and face-to-face screenings and intakes on-site and via telehealth for military family members, veterans and their families, and eligible military contractors. You will work with a multi-disciplinary team for case assignments, consultations, and care coordination. Specifically, you will:
- Serve as an initial point of contact for individuals seeking mental health services.
- Conduct initial screenings to gather demographic, clinical, and psychosocial information.
- Assess presenting concerns, urgency, and appropriateness for services based on clinical protocols and with support from Lead Clinician.
- Identify risk factors and indicators of clinical urgency or crisis and escalate concerns according to clinical protocols.
- Facilitate warm handoffs and ensure continuity of care during the intake process.
- Match clients to providers based on clinical needs, availability, and specialization. This matching and case assignment will be conducted with support from Cohen Clinic Lead Clinician or Clinic Director and multi-disciplinary team.
- Schedule initial appointments and coordinate referrals to appropriate clinicians or services.
- Collect and enter required intake documentation accurately in the electronic health records (EHR) system.
- Ensure intake notes, consents, and screening measures are completed appropriately in accordance with CARF and CVN organizational standards.
- Provide clear information about services, expectations, and the next steps.
- Support client engagement by addressing barriers to care (e.g., scheduling, access, understanding services, etc)
- Respond to inquiries with professionalism, empathy, and cultural sensitivity.
- Participate in team meetings and case consultations as appropriate.
- Maintain a comprehensive client referral tracking system. Prepare reports as requested by Clinic Director and Lead Clinician.
- Provide services in person and via telehealth platform, as needed.
- Track and report referral to client conversion rate and provide ongoing program evaluation and recommendations to the Clinic Director to aid in continuous clinic growth and quality assurance efforts.
- Knowledge of local community resources and provide warm hand-offs for veterans and families needing external referrals.
What’s required
- Master’s Degree in Social Services field to include one of the following: LMSW, LICSW, or LPC. May consider an LMSW-A or LPC-A (provisionally licensed) if eligible for full licensure within 6 months.
- Maintain active license as required by state regulations. Stay current in the field of mental health, counseling, case management, and other related social services.
- Proficient with documenting within an Electronic Health Record (EHR)
- Some travel may be required.
- Knowledge of mental illness and mental health diagnoses. Knowledge of clinical services and community resources.
- Perform other duties as assigned.
