The role

The Nurse Navigator II is Navi’s advanced clinical navigator role — supporting acute and complex clients through a blend of in-person assessments, care planning, triage support, and close collaboration with nurse contractors, support persons, and external care partners.

You will work with clients, support members, and the greater Navi team to build individualized care plans, conduct in-person visits across Maricopa County, provide real-time clinical support to nurses, and uphold quality standards through documentation, in-person and remote audits, and escalation pathways.

This role requires strong clinical judgment, autonomy, flexibility, expertise in making care recommendations, and the ability to build trust with clients and families from the very first interaction.

Note: This is a field-based hybrid role. Local travel to client sites is required (approximately 3-5 in-person visits per day), and travel to the office or occasional remote coverage may be required based on business needs. 



What you’ll do

  • Clinical Leadership & RN Support
    • Serve as an ongoing clinical resource for RN contractors by providing real-time guidance, escalation support, and evidence-based recommendations for client care decisions.
    • Provide RN quality oversight through documentation reviews, feedback, and light coaching to uphold Navi clinical standards.
    • Identify knowledge gaps and support continuous improvement through mentorship, education, and performance guidance.
    RN Onboarding & Orientation
    • Facilitate onboarding and orientation of newly hired RNs, including coordinating and conducting shadow shifts to ensure readiness for independent client care.
    • Support competency development and reinforce Navi workflows, documentation expectations, communication standards, and care philosophies.
    Client Experience & Medication Management
    • Provide in-person support during client visits, helping establish trust, assess immediate and ongoing needs, and ensure continuity of care throughout the entire care experience.
    • Oversee and support client medication management activities including reconciliation, medication education, adherence monitoring, care coordination with prescribing providers, and identification/escalation of medication-related concerns.
    Operational Coverage & Staffing Support
    • Provide flexible direct client coverage, including filling urgent or unstaffed shifts when contractor availability is limited, to ensure continuity of care and support evolving operational needs. 8-12 hours/week will be allocated to direct patient care.
    • Provide secondary cross-functional coverage and support across all Navigator roles (Intake, Remote, and Field) to ensure continuity of care and operational efficiency.
    • Identify risks or deterioration and escalate appropriately to the team lead.
    • Coordinate care across multiple teams (contractors, providers, scheduling specialists, Navi operations).
    • Maintain detailed, timely, and accurate documentation.
    • Participate in continuous improvement initiatives, nursing huddles, and QA reviews.


Who you are


  • Registered Nurse (AZ), unencumbered.
  • 3+ years of direct client care experience, including at least 1 year acute (ICU/PCU/ER strongly preferred).
  • Current AHA BLS and/or ACLS.
  • ASN or higher from an accredited nursing program.
  • Excellent interpersonal, in-person communication, and crisis-triage skills.
  • Experience precepting, mentoring, training, or orienting nursing staff preferred.
  • Strong understanding of medication management, care transitions, and quality assurance processes.
  • Comfortable balancing clinical leadership responsibilities with direct client care and operational support.
  • Proficient with Google Suite, EHRs, telehealth platforms, and mobile documentation tools.
  • Comfortable traveling within Maricopa County to visit clients and support contractors.
  • Thrives in a fast-paced, autonomous, startup culture.

Location & Hours


  • Hybrid role: primarily in-the-field, with some remote and in-office aspects
  • Local travel required for in-person client visits.
  • Must have reliable transportation
  • 36-40 hours/week, including some weekend coverage.
  • Completion of 20+ hours of in-person training with Navi leadership and/or team members.
  • Rotating After-Hours on-call coverage 
  • Training begins at Navi HQ (Phoenix), followed by hybrid flexibility.
  • Travel to office or client sites may be required for coverage.
