JOB TITLE: Patient Access Referral Coordinator
DEPARTMENT: Physician Referrals
SUPERVISED BY: Site Operations Manager
SUMMARY:
This position requires the employee to know and understand confidentiality and to employ the strictest confidentiality when handling patient information. This position assures successful arrangement and completion of patient referral documents, both internally and externally.
EDUCATION, TRAINING AND EXPERIENCE:
Minimum of two (2) years of post-high school training in office management or similar course of study and one (1) year of medical office work experience.
At least one (1) year of medical terminology.
Able to work without supervision.
Understand English grammar and office etiquette.
Possess clerical skills & able to operate office equipment (e.g. fax, copier, etc.)
Demonstrate familiarity with Microsoft office products.
Possess Organizational skills and have ability to work under deadlines.
Ability to multi-task establishes priorities, works independently and proceeds with objectives without supervision.
Clear speech and adequate hearing and vision are necessary to perform functions required in clerical and administrative activities.
JOB RESPONSIBILITIES:
Answer telephone calls. Uses telephone to make appointments for SWLACH’s patients that are referred to other providers for specialty services. Follow up on SWLACHs’ referrals to hospitals and other providers.
Complete referral forms. Mails and/or faxes forms to designated providers. Copy forms and incorporates them into the tracking system.
Notify patients of appointments and provide answers to their questions as appropriate.
Maintain a record of all outbound referrals and log them into the tracking system.
Receive patient’s records/referrals after service and forwards them to Medical Records for scanning.
Complete requests for additional information from providers receiving SWLACHS’ Referrals. Calls hospitals and physician offices to obtain additional information on referrals to SWLACHS.
Assist COO/Referral Supervisor offices with clerical activities. Assist COO with HIPAA activities as required.
Assumes additional related functions when necessary, as assigned.
