As a Federally Qualified Health Center (FQHC), our mission is rooted in providing high-quality, accessible care to all patients—regardless of their ability to pay.
CCFHS has proudly provided comprehensive office-based primary care services for more than 10 years. Our care is delivered by New Hampshire-licensed Physicians (MDs), Nurse Practitioners (NPs), and Physician Assistants (PAs), and includes the diagnosis and treatment of acute and chronic illnesses, preventive services, screenings, and health education guided by evidence-based practices. We also assess patient needs for social and nutrition services and coordinate appropriate referrals to specialty medical and behavioral health providers.
