Job Description

  • Performs initial and ongoing clinical assessments via telephone calls to clients for the purpose of assessing client's situation with regards to healthcare needs, cultural implications and support systems in place.
  • Creates a case management plan based on the assessment with measurable goals and objectives utilizing evidence-based criteria; monitors ongoing progress toward these goals and objectives; implements plan through case management interventions and communicates with all parties to reach desired goals and objectives.
  • Evaluates need for alternative treatment.
  • Applies clinical judgment to the incorporation of strategies designed to reduce risk factors and barriers and address complex clinical indicators which impact care planning and resolution of patient issues.


Qualifications

  • Must have at least 3-5 years case management experience
  • Must have 1-2 years managed care experience
  • Medicare knowledge
  • Must have Bachelors Degree in Nursing
  • Must have valid Florida RN License
  • Must be proficient with EMR

 Hours for this Position:

  •  M-F 8-5
