Job Description

Assessing patients’ ADLs (Activities of Daily Living) and doing HRAs (health risk assessments) and care plan reviews to assess the need for services that allow members to remain out of the hospital

Working with the ABD (aged, blind, disabled) population

Telephonic position located in the McAllen Superior office

Superior & TX refers to this as the “Stars-Plus“ program, which is their MMP (Medicaid/Medicare) product for dual eligible members.

Qualifications

RN license

LTC or Home Health experience

Look for experience working with the Aged, Blind and Disabled population (SSI patients on disability)

Experience coordinating services and knowledge of community/government-funded services in the area

Strong candidate will have experience coordinating services, assessing ADLs and evaluating what is needed to keep patient living safely at home/out of hospital.

Strictly clinical experience might not be strong enough for this environment.
