Job Description

ā€¢ This person will be working directly with members to assess, plan, and coordinate care

ā€¢ They will have a caseload of about 75 members!

ā€¢ Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care

ā€¢ Monday-Friday 8a-5p, Full time with medical benefits offered

ā€¢ This is an immediate Right to Hire opening!

ā€¢ Pay range $32/hr-$35/hr salary negotiated based on relevant experience


Qualifications

ā€¢ RN

ā€¢ 2+ years of clinical nursing experience in acute care setting
