Position Summary

 

Under the direction of the Lead Service Coordinator, the Community Housing Transition Worker will support First Nation citizens to acquire permanent housing. The Community Housing Transition Worker will provide advice, advocacy, and support on social and personal issues to clients that need such assistance and will organize and deliver workshops and information sessions on social and personal development.

 

Key Responsibilities

  • Interview clients to obtain case history and background information.
  • Assess client’s relevant skills and strengths.
  • Assist clients to sort out options and develop plans of action while providing necessary support and assistance.
  • Assist clients in locating and utilizing community resources, including legal, addiction recovery, medical, financial assistance, housing, employment, transportation, assistance with moves and other referral services.
  • Prepare intake reports.
  • Meet with clients to assess their progress, give support, and discuss any difficulties or problems.
  • Refer clients to other social services agencies.
  • Advise and aid recipients of social assistance and pensions.
  • Provide information on crisis intervention.
  • Implement and organize the delivery of specific services within the community.
  • Implement life skills workshops, substance abuse treatment programs, behavior management programs, youth services programs and other community and social service programs.
  • Assist in evaluating the effectiveness of treatment programs by tracking client’s behavioral changes and responses to interventions.
  • Maintain contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client’s overall progress.
  • Maintain program statistics for purposes of evaluation and research.
  • Visit homeless camps to support the residents to obtain permanent housing.
  • Other duties as assigned.

 

Qualifications

Education & Experience

  • Completion of a diploma program in social work, child and youth care, counseling or other social science or health related discipline, equivalent combination of education and experience may be considered.
  • Extensive work experience in a social service environment as an employee, volunteer or in a support capacity, may replace formal education requirements.
  • Understanding of trauma-informed practice and harm reduction principles.
  • Knowledgeable of First Nation cultural values and traditions.
  • Experience interacting with a diverse group of individuals.
  • Demonstrated organizational and time management skills.
  • Able to set priorities and work under pressure.
  • Excellent verbal and written communication skills.
  • Team player with strong interpersonal skills, ability to work with staff and the public, outgoing and friendly personality.
  • Demonstrated ability to initiate, attention to detail and maintain work accuracy.
  • Punctual and reliable, able to follow directions in a timely fashion.
  • Working in a confidential work setting is preferred.
  • Proficient in using Microsoft office -Word, Excel, Power Point and Outlook.
  • Adhere to AMC policies, and procedures with specific attention to attendance, and professional ethics.
  • Must be able to work flexible hours and be able to travel upon request.
  • Must have a valid Class 5 Driver’s License, and a reliable vehicle.
  • Must have a current satisfactory Criminal Record Check, Clear Adult and Child Abuse Checks.
