About this role

TASKS AND RESPONSIBILITIES



  1. Treat all individuals with dignity and respect while maintaining agency standards of confidentiality, professionalism, and objectivity. 
  2. Maintain case management and oversight of the IOP (Intensive Outpatient) population & Offer continuity of care by providing step down support and services into a specified EOP (Extended Outpatient) milieu.
  3. Provide intervention, evaluation/assessment, individual, group and family counseling to individuals affected by substance use disorders and secondary co-occurring disorders.
  4. Create individualized safety planning and person-centered treatment planning to all patients in the program.
  5. Develop and maintain a community referral network.
  6. Provide education and training to individuals, groups, and agencies.
  7. Consistent collaboration and coordination of care between all Prairie Ridge services.
  8. Participate in patient transition & discharge plans, including development of relapse prevention and wellness plans, scheduling, and aftercare placement.
  9. Engage in crisis intervention and risk management as appropriate within the program.
  10. Develop and maintain a working knowledge of current and emerging evidence-based practices and approaches and incorporate in group curriculum and individual therapy.
  11. Attend staff consultation, supervision, and training activities.
  12. Aim to uphold and contribute to agency culture, mission, vision, and value.
  13. Proficiency in clinical practices identified as best practice by the agency demonstrated through supervision and direct observation.
  14. Develop and maintain a working knowledge of all documentation and reporting systems to include ISmart, Accreditation Now, MyOutcomes, and Electronic Health Record.
  15. Maintain all regulatory, contract and documentation standards and data collection, insurance authorization requirements, and complete necessary documentation in a timely manner.
  16. Demonstrate understanding and adherence to all CARF policies and procedures.
  17. Provide adjunct support and coverage to the Residential & Full Day Programs.
  18. Assume 24hr on-call duty once every clinical staff rotation.
  19. Perform other duties as assigned by supervisor.

 

QUALIFICATIONS

 

  1. For clinicians treating individuals with a primary substance use disorder diagnosis, an Iowa Substance Abuse Counselor Certificate or license preferred. For clinicians treating individuals with a primary mental health diagnosis, a minimum of a master’s degree in behavioral healthcare and a license as a LMFT, LMHC, LMSW, LISW, or psychologist
  2. Bachelor's Degree in health-related field required for treatment of substance use and gambling disorders: Master’s Degree in behavioral healthcare field licensable in Iowa required for treatment of a primary mental health diagnosis. 
  3. Understanding of the mission of Prairie Ridge and the patients we work with.
  4. Proficiency in oral and written communication skills, as well as record keeping.
  5. Technology skills allowing for proficiency in use of electronic health record.
  6. Physical demands: The employee frequently lifts and moves up to 10 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus.
