About this role

Job purposeĀ 

The purpose of the Case Manager position is to support children and/or adults with Serious Mental Illness (SMI) in achieving stability, safety, and improved functioning by coordinating services,Ā monitoringĀ progress, and advocating for needed supports. The Case Manager ensuresĀ timelyĀ engagement, consistent service delivery, andĀ accurateĀ documentation in compliance with agency standards, state regulations, and billing requirements. Through client-centered planning, collaboration with families and providers, and connection to community resources, the Case Manager promotes continuity of care and helps clients progress toward their individual goals.Ā 

Duties and responsibilitiesĀ 

  • Engage clients and families in ongoing, client-centered case management services that support stability, safety, and progress towardĀ identifiedĀ goals.Ā 
  • Schedule all client appointments monthly in Empathic as billable services to ensure required contact and billing standards are met.Ā 
  • Contact new clients within 1–2 business days to complete intake and schedule theĀ initialĀ appointment.Ā 
  • Monitor attendance and makeĀ timelyĀ efforts to reschedule missed or canceled appointments within the same month.Ā 
  • Complete,Ā maintain, and update required assessments and service plans, including Functional Assessments, Crisis Plans, Individual Family and Community Support Plans (IFCSP), CASII assessments, and Diagnostic Assessments, within required timelines.Ā 
  • Document all client, collateral, and provider contacts accurately and within three business days, including attempted contacts and missed appointments.Ā 
  • Coordinate andĀ monitorĀ services across the local system of care, including mental health providers, schools, social services, and community resources.Ā 
  • Participate in regular supervision to review service delivery, documentation, and compliance with agency and state requirements.Ā 
  • Submit Requests for Service Authorizations andĀ requiredĀ documentation at least 30 days prior toĀ expiration.Ā 
  • Follow agency protocols for client discharge, transitions, and case closures.Ā 
  • Maintain a caseload ofĀ approximately 12–15 clients and provide coverage for other Case Managers as assigned.Ā 
  • Refer clients to crisis services when emergencies occur outside of business hours,Ā in accordance withĀ agency policy.Ā 
  • Attend requiredĀ trainingsĀ and complete supervision, mentoring, and continuing education requirements.Ā 
  • Track andĀ submitĀ mileage and parking expenses within required timelines.Ā 
  • Adhere to ethical standards, confidentiality requirements, and agency policies.Ā 
  • Case Managers do not transport clients or family members in personal vehicles.Ā

QualificationsĀ 

CandidatesĀ must meetĀ one or moreĀ of the following criteriaĀ in accordance withĀ Minnesota statute:Ā 

  • Experience and training in working with children with mental health needs.Ā 
  • Qualify as aĀ Mental Health PractitionerĀ under Minn. Stat. §245I.04, subd. 4;Ā ORĀ 
  • Hold a bachelor’s degree in a behavioral science or related field (including, but not limited to, social work, psychology, nursing) from an accredited college or university;Ā ORĀ 
  • Meet one of the alternative qualification pathways outlined below.Ā 

Applicants who are not mental health practitioners and do not hold a qualifying bachelor’s degree may qualify if they meetĀ oneĀ of the following:Ā 

  • Three or four years of experience as aĀ Case Manager Associate (CMA);Ā 
  • Registered Nurse without a bachelor’s degree with at leastĀ three yearsĀ of combined psychiatric training and relevant community-based mental health experience;Ā 
  • Previously qualified as a case manager under theĀ 1998 Department of Human Services waiverĀ and meet ongoing supervision and training requirements;Ā 
  • Completion of at leastĀ 80 hours of specialized trainingĀ on the characteristics and needs of children with Serious Mental Illness (SMI) prior to providing direct services;Ā 
  • Demonstrated competency in practice and knowledge of children’s Serious Mental Illness consistent withĀ national practice standardsĀ prior to service delivery.Ā 

Supervision, Training + Continuing Education Requirements:Ā 

  • Case Managers withĀ 2,000 or more supervised hoursĀ delivering mental health services to children must receiveĀ 38 hours of supervision annually, including at leastĀ one hour per month of clinical supervision.Ā 
  • Case Managers without 2,000 supervised hours must:Ā 
  • CompleteĀ 40 hours of DHS-approved trainingĀ prior to providing services; andĀ 
  • ReceiveĀ weekly clinical supervisionĀ from a mental health professional until the experience requirement is met.Ā 
  • Case Managers who are not licensed, registered, or certified by a health-related licensing board must completeĀ 30 hours of continuing education every two yearsĀ in serious mental illness and mental health services.Ā 
  • All clinical supervision must beĀ documented in the child’s record.Ā 

Case Manager Associate (CMA) Eligibility (If Applicable)Ā 

  • Must be at leastĀ 21 years of age.Ā 
  • Must have a high school diploma or equivalent.Ā 
  • Must work under the direction of a qualified Case Manager or Case Management Supervisor.Ā 
  • May qualify with an associate degree in behavioral sciences/human servicesĀ orĀ qualifying experience as defined by statute.Ā 

Additionally, all candidates must:Ā 

  • Have experience and training inĀ identifyingĀ and assessing a wide range of children’s needs.Ā 
  • Be knowledgeable about local community resources and how to access and coordinate those resources for the benefit of children and their families.Ā 
  • Meet all requiredĀ supervision, training, and continuing education requirementsĀ as applicable.Ā 

Pay:

$24-$26 per hour
