Hours of Work/Schedule: 35 Hours per week (May includes some evening and weekends)

Position Reports To: Manager EPESI 

Salary Range: Annualized salary range $75,000 – $90,000, commensurate on skills and experience, plus participation in group benefits and HOOPP.

Primary Work Location: Virtual, 115 Grassmere Ave. South Oshawa site) and across various agency locations within Durham Region.

Durham Community Health Centre (DCHC) is a registered, charitable organization that provides integrated, accessible, and equitable community-based primary care, wellness services, and health education to Durham community members who face multiple barriers to their health and well-being.

DCHC also provides equity-based programs and services that focus on priority populations such as Indigenous, Black, the 2SLGBTQI Community, Newcomers to Canada, Seniors, and Unattached patients (i.e., those without a family doctor), to name a few. DCHC ensures Durham community members receive not only sick care, but preventive care as well.

At DCHC, we are a team-based interprofessional group of staff including physicians, nurse practitioners, nurses, counselors, dietitians, outreach workers, medical secretaries, and other administrative staff. We always place every client at the Centre of our approach to care, based on their needs. We address these needs through integrated clinical and wellness care and health education.

Position Overview

The Project Specialist will support the delivery of strategic initiatives across Durham Community Health Centre by providing coordinated project support, alongside supporting evaluation, performance monitoring, implementation, and change management activities.

Working within the EPESI team, the role will contribute to strengthening a consistent and structured approach to project delivery across the organization. This includes supporting planning, implementation, monitoring progress, assessing outcomes, and supporting adoption of new processes and frameworks through effective stakeholder engagement and change management approaches to ensure initiatives are aligned with organizational priorities and deliver measurable impact.

The role is intended for a highly organized and analytical professional who can integrate project delivery, evaluation, implementation support, and performance monitoring within a complex and evolving healthcare environment.

 

Duties and Responsibilities

Key Responsibilities

1. Project Coordination & Delivery

·        Support the planning and delivery of projects and cross-functional initiatives

·        Develop and maintain project plans, timelines, milestones, dependencies, and implementation trackers

·        Track progress against deliverables and timelines, identifying risks, issues, and mitigation actions

·        Maintain project documentation, including risk, issue, decision, and action logs

·        Coordinate meetings, workshops, follow-ups, and implementation activities across teams

2. Evaluation & Performance Monitoring

·        Support the design and implementation of project and program evaluation approaches

·        Work with program leads to define outcomes, KPIs, reporting measures, and data collection processes

·        Support development of dashboards, reports, evaluation summaries, and performance updates

·        Analyze and interpret data to assess progress, outcomes, risks, trends, and areas for improvement

·        Contribute to organizational learning, continuous improvement, and evidence-informed decision making

3. Governance, Reporting & PMO Support

·        Support consistent project governance and reporting practices across initiatives

·        Prepare briefing notes, status reports, executive summaries, and presentation materials

·        Support maintenance of project registers, reporting templates, and implementation tools

·        Contribute to enterprise performance reporting and strategic initiative tracking

·        Escalate risks and implementation concerns in a timely manner with proposed mitigation actions

4. Change Management & Implementation Support

·        Support implementation of new processes, tools, frameworks, and ways of working across the organization

·        Work collaboratively with teams to support adoption and consistent use of project management, evaluation, and reporting practices

·        Identify and help address barriers to implementation and uptake

·        Contribute to communication, engagement, and training approaches that support change initiatives

·        Support sustainability of improvements through follow-up, monitoring, and continuous improvement activities

5. Stakeholder Engagement & Strategic Support

·        Liaise with internal departments and external partners to support coordination and delivery

·        Maintain effective communication with stakeholders across multiple initiatives

·        Support alignment of projects and implementation activities with organizational priorities

·        Contribute to strengthening a coordinated and structured approach to strategic initiative delivery within EPESI


Qualifications & Experience

Qualifications:

·        University degree in a related discipline (e.g. Health Administration, Public Health, Business Administration).

·        Recognized project management certification (e.g. PMP, PRINCE2, Agile).

·        Formal training or certification in change management (e.g. Prosci).

·        Minimum 5 years’ experience in project coordination, project management, implementation, or program delivery roles within a complex environment

·        Experience working in healthcare, community health, public sector, or non-profit settings with reporting requirements to funders and leadership

·        Demonstrated experience supporting and implementing projects and programs with defined outcomes, performance measures, reporting requirements, and change management components.  Including adoption of new processes, tools, frameworks, or continuous improvement methodologies (e.g. Lean Six Sigma)

·        Demonstrated experience applying evaluation and performance monitoring methodologies, including process and outcome evaluation, and using logic models, outcomes frameworks, or implementation frameworks to support continuous improvement.

·        Experience developing and supporting KPIs, dashboards, data collection processes, and organizational performance reporting, with the ability to interpret and communicate data to support operational and strategic decision-making.

·        Experience coordinating across multiple stakeholders or departments with competing priorities and timelines

·        Proficiency with project management and collaboration tools such as Microsoft Project, Jira, Asana, Trello, Microsoft Teams, and SharePoint

·        Strong organizational and coordination skills

·        Ability to manage multiple priorities and timelines effectively

·        Ability to facilitate meetings and prepare reports, briefing materials, and presentations

·        Strong analytical, evaluation, and problem-solving skills, with the ability to interpret, synthesize, and present data effectively.

·        Strong communication, stakeholder engagement, and interpersonal skills, with the ability to support organizational change and adoption of new ways of working.
