Summary:

The Community Health Worker (CHW) provides coordination of care for patients and families between ACH primary care providers and appropriate community resources. The CHW will help families navigate various systems of care while connecting them to the medical, social, and mental health services for which they may be eligible. CHWs serve as a liaison between the family, providers, and community based organizations (CBO). They should have a strong understanding of the healthcare delivery system and help patients overcome access challenges. The employee will provide coaching, support, and guidance to the patient as they work towards their health care goals.

Additional Information:

Required Education:

Bachelor's degree from four-year college or university; Or four years related experience and/or training; or equivalent combination of education and experience.

Description

1. Coordinate with primary care treatment team and family to address social determinants of health. Β 

2. Work closely with clinic Social Worker and/or Behavioral Health Clinician to reinforce treatment recommendations with patient in community settings Β and facilitate access to resources and referrals.Β 

3. Build relationships with local CBOs, outpatient providers, acute or residential psychiatric treatment facilities to support families throughout the process of accessing appropriate medical, psychosocial, and mental health services.Β 

4. Provide health education to patients and families on lifestyle and behaviors that promote wellness.

Β 5. Coordinate with providers and payers to help patients access the correct level of care in order to avoid healthcare crises.Β 

6. Assists patients with enrollment in programs and benefits for which they are eligible.

7. Follows-up on referrals, resources, and recommendations to help address access barriers as they arise.Β 

8. In compliance with Arkansas Children's Fleet Management Policy drives an owned, leased, or personal vehicle frequently in the performance of essential job functions.
