Job Summary

Join a Mission-Driven Team at the Mass General Cancer Institute (MGBCI) as a Patient Navigator!

Essential Functions:

* Lead and participate in the intake process for new patients, helping them navigate their journey into MGBCI with compassion and clarity.

* Provide essential case management services—including financial assessments, scheduling, and medical records coordination—to support patients and their families.

* Ensure timely and accurate documentation in the electronic medical record, meeting expectations set by our Medical Director and Administrative Manager.

As a Patient Navigator, you'll be at the forefront of patient advocacy, helping individuals access the care they need while working alongside a dedicated and collaborative team. Your contributions will directly support MGBCI’s mission to deliver world-class cancer care with empathy and excellence.

Responsible for working closely with patients, families, and healthcare providers to ensure a seamless and patient-centered care experience by assisting with navigating the complexities of the healthcare system, coordinating resources, and providing support to improve patient outcomes.

Advocate for patients and their families, ensuring their needs and preferences are considered in the care planning process.

Serve as a liaison between patients, healthcare providers, and other relevant stakeholders.

Collaborate with the interdisciplinary healthcare team to coordinate patient care services, appointments, and follow-up plans.

Assist patients in understanding and adhering to their care plans.

Identify and connect patients with appropriate healthcare and community resources, such as support groups, financial assistance programs, and transportation services.

Educate patients and their families about their medical conditions, treatment options, and self-care strategies.

Ensure that patients are informed and empowered to actively participate in their healthcare decisions.

Maintain accurate and detailed documentation of patient interactions, care plans, and resource referrals.

Bachelor's Degree required

- Strong knowledge of healthcare resources, community services, and patient advocacy.
- Excellent communication and interpersonal skills.
- Ability to collaborate effectively with healthcare professionals across multiple disciplines and experiences.

- Familiarity with electronic health records and case management software.
