Job Description:

GENERAL DESCRIPTION:

The mission of Metrocare Services is to serve our neighbors with developmental or mental health challenges by helping them find lives that are meaningful and satisfying.  We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families and adults we serve.  

The IDD Jail/ Court Liaison is a position with in the Local Intellectual Developmental Disability Authority (LIDDA) who works with individuals with a primary diagnosis of intellectual or Developmental Disability who are involved or at risk for involvement in the criminal justice system.  Individuals may also be court ordered for treatment, jail diversion and outpatient competency restoration programs.  The Liaison also coordinates services for individuals with IDD committed to the State Hospital System. This position is customer service oriented and works closely with other departments within the agency, community stakeholders and Legal Authorized Representatives (LAR) or Actively Involved Persons in the individual’s life. 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The essential functions listed here are representative of those that must be met to successfully perform the job.

Meet with IDD individuals at their homes, in the community, at jails or juvenile detention centers, or at the state hospital to ascertain their needs and help them determine what services they require.
Collaborate with individuals/LAR/Actively Involved Person to identify needs and develop individualized, person directed plans that reflect individuals’ goals and objectives. Review and update the plan as needed.
Help individuals access services to help them achieve the quality of life they desire.
Meet with individuals at least monthly to ensure they are getting needed services, to evaluate the effectiveness of the services, and to determine new problems and new interventions. 
Collaborate with IDD/MH service providers to discuss service delivery options for individuals served while linking, monitoring and advocating to ensure service needs are met.
When a crisis arises, be available to help link and assist individuals to secure services that will enable them to prevent future crisis.
Complete paperwork requirements and maintain quality, up-to-date clinical records. Document any clinical or support services within 24 business hours of providing the service.
Link individuals to resources and a system of natural supports.

Collaborate with external agencies, including the Local Mental Health Authority (LMHA), juvenile detention centers, and community programs, to identify and connect IDD individuals with services.
Attend and participate in all scheduled team meetings.
Facilitate Outpatient Competency Restoration Program (OCR) as assigned by the courts.
Complete State Supported Living Center (SSLC) application and Home and Community-Based Services (HCS) Crisis Diversion application for 46B Commitments as need.
Accompany and advocate for individuals during court hearings.
Effectively communicate with individual’s support team.
Complete Texas Law Enforcement Telecommunication System (TLETS) tasks and reports.
Collaborate with local law enforcement and educate on LIDDA services for individuals with IDD.

Support the IDD Crisis Intervention Team as needed.
Perform other duties as assigned.

QUALIFICATIONS

EDUCATION AND EXPERIENCE:

A bachelor’s or advanced degree from an accredited college or university with a major in a social, behavioral, or human service field, including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice. 


Master’s preferred


At least one (1) year experience working with individuals with IDD/DD and Mental Illness

MATHEMATICAL SKILLS:

Basic math skills required.
Able to work with reports and numbers.
Able to calculate moderately complex figures and amounts to accurately report activities and budgets.
                                               
REASONING ABILITY:

Able to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with only occasional or no variables.

COMPUTER SKILLS:

Use computer, printer, and software programs necessary to the position (i.e. Word, Excel, PowerPoint, Outlook, and EHR).
Ability to utilize Internet for resources.

CERTIFICATIONS, LICENSES, REGISTRATIONS:

Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.
Liability insurance required if employee will operate personal vehicle on Center property or for Center business.  Must be insurable by Center’s liability carrier if employee operates a Center vehicle or drives personal car on Center business.  Must have an acceptable driving record.
