The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

The senior social worker collaborates with the Social Work Clinical Coordinator and Clinic Director in the ongoing management and operations of Adult Ambulatory Crisis and Access coordination. They serve as a member of the Leadership Team, participating in the development, implementation, and ongoing evaluation of initiatives to effect integrated, collaborative, patient-centered, recovery orientated care. In this role, they also coordinate timely access to care for individuals referred for services. The senior social worker is also responsible for the clinical and administrative supervision of assigned social workers, LMHC’s, LMFT’s, or other disciplines as appropriate. Last, the senior social worker is also responsible for providing clinical services for patients including the provision of individual, family, and group therapies in addition to crisis intervention and community outreach. This is provided through the participation on an interdisciplinary team designed to provide comprehensive community based psychiatric care.

ESSENTIAL FUNCTIONS

  • As a member of an interdisciplinary team, provides direct clinical services to assigned patients, which may involve individual, family, and/or group therapy, crisis intervention, and diagnostic evaluations.  Engaged in consultation, treatment planning, and periodic care review as well as collaborates with family, social supports, community agencies (past and present) to ensure appropriate and effective care.  Coordinates referrals and facilitates linkage to other appropriate agencies/supports.
  • Provides administrative and clinical supervision to therapists                      consistent with the Clinic and Department standards.
  • As a member of the Adult Ambulatory leadership team, participates in policy and procedure development for the Clinic, establishment of initiatives to improve positive patient care outcomes, and provides teaching to Clinic staff.
  • Provides telephone screening and assessment to individuals wanting to access services at Adult Ambulatory Clinic.  Maintains real time handle on clinic schedules and flow of patients through the system
  • In partnership with the Social Work Clinical Coordinator, facilitates interdisciplinary treatment team meetings as well as other general staff/therapist meetings
  • Assists in recruitment and evaluation of performance of therapists including
  • interviewing, quarterly audits of the electronic medical record, and yearly
  • performance evaluations
  • Documents interventions, both direct and indirect, in the patient’s medical record in compliance with the Division, Department, and Hospital policy.
  • Attends Division, Department, and Clinic level staff meetings. Ensures that Division, Department, and Clinic policies and procedures are implemented with regard to professional practice and patient care.  Ensures timely completion of statistical reports/forms, time sheets, and other Hospital or Division mandatory in-service trainings and Health Assessments.  Subscribes to maintaining a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties.

MINIMUM EDUCATION & EXPERIENCE

  • Master's degree from an accredited school of Social Work and 2 years of professional social work practice required
  • Or equivalent combination of education and experience


LICENSES AND CERTIFICATIONS

  • LMSW - Licensed Medical Social Worker - State Licensure as defined by the State Board for Social Work, State Education department, Division of Professional Licensing Services upon hire required
