Position Overview 

Are you passionate about creating safer communities and reducing violence? We are seeking a dedicated and compassionate Client Case Manager to join our violence prevention and intervention S.O.L.V.E team. In this vital role, you will focus on assessing client needs, developing customized case plans, and providing or connecting individuals to crucial resources and services,
including both perpetrators of gun violence and victim services. Our programming and services
encompass a variety of support avenues, including effective emotional or trauma-related therapies, legal aid services, housing services, employment training, job placement services, family engagement, re-entry services, and much more. 

 

As a Client Case Manager, you will work closely with individuals to understand their unique situations and develop strategies that empower them to overcome systemic challenges, such as those posed by gun violence. This full-time position offers you the opportunity to make a tangible difference in the lives of our participants. By emphasizing collaboration with community resources and workforce development programs, we ensure that our support is both timely and effective. Responsibilities 

  • Conduct comprehensive intake assessments to identify client needs. 
  • Develop tailored case plans to guide clients in achieving their goals. 
  • Provide or facilitate access to a variety of services, including:  
  • Emotional or trauma-related therapies  
  • Legal aid and housing services  
  • Employment training and job placement assistance  
  • Family engagement and re-entry services  
  • Additional relevant support services  
  • Build trust with clients through effective communication and interpersonal skills. 
  • Ensure participants complete intake forms, maintaining precise and detailed case records. 
  • Collaborate with workforce development programs to provide participants with timely opportunities. 
  • Manage a caseload of up to 25 participants at any time. 
  • Contribute to the organization's efforts in gun violence prevention and community safety. 
  • Connect participants with essential workforce development programs. 
  • Ensure services are provided only to eligible participants through completed intake forms. 
  • Join a mission-driven team dedicated to community empowerment and gun violence prevention. 

 

Qualifications 

 

  • Bachelor’s degree in social work, psychology, counseling, or a related field (preferred). 
  • Proven experience in case management, social services, or a similar field. 
  • Strong communication and relationship-building skills to establish trust with clients. 
  • Knowledge of community resources and an ability to effectively facilitate access to services. 
  • Proficiency in assessing client needs, developing actionable case plans, and monitoring progress. 
  • Familiarity with issues surrounding mental health, substance abuse, domestic violence, and the criminal justice system. 
  • Competent in record-keeping and case documentation. 
  • Ability to work both independently and collaboratively within a supportive team environment.

Work Environment: 

This position is multi-site, meaning that work generally takes place in multiple locations and requires independent travel. 

 

Mental and Physical Requirements: 

Physical effort is light, with lifting or carrying limited to 25 pounds intermittently as well as walking up and down stairs. Occasionally, there is pressure associated with project or activity deadlines
