Company Summary: Crossroads is an Arizona Department of Health Services licensed substance abuse treatment provider with proven expertise in serving men, women and Veterans with Substance Use Disorder through the provision of high quality, successful, and affordable residential and outpatient recovery programs.

Job Summary: Under supervision of Residential Clinical Program Manager, the Clinical Coordinator will perform professional work by providing clinical intervention services to an adult population composed of substance-abusing referred persons in a residential and outpatient setting of care. The Clinical Coordinator is responsible for screenings, assessments, treatment planning, case load management, group/individual interventions, crisis management and coordination of care independently, with limited supervision.



Supervisory Responsibilities: 

  • Recruits, interviews, hires, and trains employees.
  • Provides constructive and timely performance evaluations.
  • Recommends compensation adjustments.
  • Handles discipline and termination of employees in accordance with company policy.
  • Involved with handling all employee relation issues.
  • Timecard responsibilities.
  • PTO management.



Essential Functions:

  • Oversees all aspects of the facilities clinical team, including assignment of caseloads, scheduling staff, provides support and training when necessary, and conducts audits of client records to ensure documentation is complete and accurate.
  • Conducts assessments or screenings with clients to evaluate the nature of their presenting problems in order to determine their needs and develop appropriate treatment plan goals.
  • Develops and provides individual treatment plans; monitors, evaluates and documents client’s progress.
  • Prepares intake documentation, progress notes, and all other documentation regarding clients in accordance with regulatory requirements and Crossroads policies and procedures.
  • Monitors clinical staff caseloads daily to ensure required documentation is completed timely; including, but not limited to, assessments, treatment plans, progress notes, case reviews and discharge plans.
  • Provides individual and group interventions to include crisis management as required.
  • Facilitates coordination of care to include case review, therapeutic interventions, client progress in treatment and coordination of aftercare services.
  • Participates in clinical staff meetings, trainings and other activities as required.
  • Assists with clinical training with team members.
  • Perform other duties, as required.



Qualifications:

  • Must meet Arizona Department of Health Services personnel member requirements of: at least 21 years old; or licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice.
  • Current CPR/First Aid/Narcan training.
  • Current and annual negative TB Test.
  • Current and valid Fingerprint Clearance Card.
  • Clean Motor Vehicle Record (MVR) check performed annually and upon hire, preferred.
  • Successfully pass a background check and an Arizona Department of Health Services Adult Protective Services check.



Knowledge, Skills, and Abilities:

  • Provide strong leadership qualities as well as problem solving skills.
  • Demonstrated strong organizational skills.
  • Ability to communicate effectively at multiple levels, both verbal and written.
  • Ability to work independently and as a member of a team.



Education & Experience:

  • Minimum of a High School Diploma or GED required.
  • Bachelor’s degree preferred.
  • 1-2 years behavioral health experience.
  • Any equivalent combination of education and experience.



Work Environment:

  • Fast paced behavioral health setting.
  • Primarily indoors, however, some outdoor
    work may be requested.



Physical Demands:

  • While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds and seldom lift and/or move >100 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.



Shift Requirements: 

  • Days, nights, weekends, and holidays.



Travel:

  • Valid AZ Driver’s License.
  • May include travel between sites and other locations.
  • Local and statewide travel when requested or required.
