Job Summary
As a Physical Therapist (PT), you are key to helping us provide superior patient care. You will work to provide skilled Physical Therapy services in accordance with the client’s plan of care to include helping clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, water, light and electricity.
We are looking for a team player who is eager to learn, passionate about providing quality care, and has a proven track record of compassionate care.
What you will do in this role:
- Under the direction of a Registered Physical Therapist (PT) provide client services which have been delegated by the PT- which includes: assist in evaluation and development of a rehabilitative plan of care and in periodic re‐evaluation as required
- Participate in the preparation of clinical and daily progress notes, all notes are reviewed by the PT and submitted within 72 hours, also prepares periodic summary reports
- As directed, perform routine strengthening and therapeutic exercises and assists the client with the use of self‐help devices
- Together with the PT, instruct the client and family and assists in teaching exercise programs to the client and family
- Consult with PT regarding the outcome of home visits and reports physical findings in written form
- Assist in evaluation and development of the rehabilitative plan of care and in periodic re‐evaluation as indicated
- Write clinical/progress notes on the date of the visit and incorporate the same in the client's record within 72 hours
- Participate in case conferences, therapy staff meetings, agency activities and in‐service programs as necessary
Qualifications
- Must be a graduate of an approved school giving a course of not less than two years for Physical Therapy Assistants
- Must be licensed or registered in Virginia
- Must have at least one year of institutional/home health experience
- Must have current CPR certification
- Working knowledge of Medicare regulations and guidelines is a plus
- You must be able to provide two references
- You must be able to pass OIG checks, Virginia State Police background check, and national background checks, and able to verify employment eligibility with necessary documentation
- Reliable transportation, proof of valid car insurance, and a valid driver’s license
