Position Summary
The Care Coordinator will play a vital role in supporting the health, well-being, and independence of an elderly couple residing in West Bay, Grand Cayman. This role combines clinical expertise, organizational oversight, and serving as a compassionate family liaison. The Care Coordinator will ensure medical care is coordinated across multiple providers, medications are reviewed and managed, and caregiving staff are effectively supervised. He or she will also support household logistics, transportation to appointments, and family communication, acting as the central point of coordination for both medical and personal care needs.
This is a senior-level caregiving position requiring medical knowledge, experience with geriatric patients and convalescent care planning, strong interpersonal skills, and the ability to manage both professional staff and sensitive family dynamics. The focus will primarily be on coordinating and providing direct care and support for the elderly gentleman in the household, with attention to the following priorities:
- Developing and implementing a convalescent care plan for the elderly gentleman’s recovery and improved independence.
- Ensuring attentive management and consistent oversight of complex medical needs.
- Close monitoring of medications, nutrition, and specialist care.
- Leading the caregiving team to deliver consistent, high-quality, supportive care.
- Safeguarding comfort, dignity, and quality of life for both the husband and wife.
- Providing regular updates to extended family members to ensure transparency and shared decision-making while maintaining continuity of care and alignment with both the couple’s preferences and the family’s expectations.
Key Responsibilities
Care Management and Medical Oversight
- Develop, implement, and monitor a convalescent care plan for the elderly gentleman.
- Establish measurable recovery goals with short, medium, and long-term milestones to guide progress and ensure the plan adapts to the pace of recovery.
- Assess personal care needs and provide the appropriate level of assistance with activities of daily living (including safe lifting assistance), and adjust support as independence improves.
- Conduct regular assessments of physical, nutritional, and emotional well-being of both the husband and wife.
- Integrate family and caregivers into the care process by clarifying roles, offering training where needed, and aligning their support with recovery goals.
- Attend and actively participate in medical appointments, taking detailed notes, asking informed questions, and ensuring follow-up actions.
- Coordinate with physicians, specialists, pharmacists, and allied health providers to ensure execution of care plans and raise concerns regarding potential conflicting guidance.
- Ensure proactive management of emerging health concerns, including nutrition and swallowing challenges, pain management, COPD, and Parkinson’s-related symptoms.
- Manage medication schedules, including monitoring for side effects, interactions, and efficacy to bring to the attention of physicians and the pharmacist for periodic medication reviews.
Staff Supervision and Coordination
- Oversee and coordinate the daily activities of full-time and part-time nurses and personal support staff to ensure proper execution of recovery objectives and daily care plan.
- Ensure effective shift handovers, clear communication, and consistent standards of care.
- Implement checklists, logs, or digital tracking tools to improve accountability and continuity.
- Provide coaching and guidance to caregiving staff, ensuring adherence to care plans, nutritional plans, and best practices.
Family and Household Liaison
- Serve as the central communication point between family members, reducing stress and duplication of effort.
- Establish a respectful working relationship with the elderly couple, particularly balancing the spouse’s involvement in care while ensuring her contributions are guided, acknowledged, and supported.
- Support social engagement for both spouses by organizing small outings or in-home gatherings.
- Provide transportation for medical appointments, errands, and occasional social activities.
- Assist with household coordination tasks when relevant (not routine cleaning, but logistics, errands, or light oversight).
Compliance, Safety and Quality of Care
- Monitor home safety, including fall risks, and infection control.
- Ensure all care follows medical guidance and aligns with the appropriate health and safety regulations.
- Oversee meal preparation and nutritional planning in alignment with the convalescent care plan.
- Respond calmly and effectively to crises or unexpected changes in health.
- Maintain meticulous documentation of care activities, medications, appointments, and observations.
Qualifications
- Licensed Registered Nurse (RN), Licensed Practical Nurse (LPN), or qualified Social Worker with healthcare coordination experience.
- Minimum 3–5 years of experience in geriatric care, community health, or home-based care coordination.
- Strong knowledge of convalescent care, chronic disease management, nutrition, and medication monitoring.
- Valid driver’s license and clean driving record (ability to transport clients).
- Excellent interpersonal, communication, and conflict resolution skills.
- Strong organizational and time-management abilities, with demonstrated leadership in supervising care teams.
- Proficiency in Microsoft Office, care documentation tools or similar systems.

Compensation & Terms
- Salary commensurate with qualifications and experience (anticipated range KYD $60,000–$80,000 annually).
- Standard hours: Monday–Saturday (half-day Saturday), with flexibility for emergencies. Position is non-live-in (at family’s request) but requires consistent availability and commitment.
