Shape the Future of Community-Based Care

This is a chance to lead the programs that keep older adults and people with disabilities living in the communities they call home. As Director of Care Management, you'll provide strategic leadership and oversight for our Care Management Department—including the MI Choice Medicaid Waiver Program and Community Transitions Program, with room to grow into new programs as we expand our impact.

You'll set the direction, build the partnerships, and develop the team that makes it all work. If you're an experienced leader in long-term care or HCBS who wants to build something lasting, we'd love to talk.

What You'll Do

  • Lead the department. Oversee program operations to meet contractual, accreditation, and agency goals while staying in strict compliance with MDHHS and federal standards.
  • Develop your team. Supervise and mentor the MI Choice Manager, Vendor Contracts and Compliance Manager, and Community Transitions Social Workers—building a high-performance, collaborative culture grounded in continuous learning and autonomy.
  • Drive quality. Evaluate program operations against quality protocols, design remediation plans, and continuously improve how we serve participants.
  • Build partnerships. Cultivate relationships with other organizations to extend the reach and impact of our programs.
  • Represent TIC. Participate in statewide and community meetings, events, and conferences, bringing insights back to the team.
  • Align with the mission. Collaborate across departments and contribute to strategic decisions about the direction of our programs.

This role isn't all-inclusive—you'll also take on related responsibilities as the work evolves.

What You Bring

  • A bachelor's degree in health and human services (social work, psychology, nursing, etc.), public health administration, or a related field
  • Five or more years in a management role in a social service setting, focused on long-term care or HCBS
  • Comprehensive knowledge of Medicare, Medicaid (especially MI Choice Waiver and/or Community Transitions), and aging/disability resources
  • Experience with person-centered planning and addressing barriers to community discharge
  • A proven track record supervising and motivating staff, mediating conflict, and solving complex problems
  • Strong communication skills—able to make complex ideas clear and convey why the work matters
  • Competence in Microsoft Office (Outlook, Word, Excel) and comfort overseeing digital tracking systems like COMPASS and CHAMPS
  • A commitment to confidentiality in line with HIPAA and agency policy
  • A valid Michigan driver's license, auto insurance, and reliable transportation

Preferred: Michigan license as an LBSW, LMSW, or RN
