JOB SUMMARY:

Performs routine clerical and clinical tasks within the Four Points Clinic, while assisting the Primary Care Provider and other licensed providers. Provides quality patient care efficiently and cost effectively at the direction of the Primary Care Provider. Works collaboratively with clinical team to ensure that excellent care is provided every day.

Monday - Friday; 8:30 AM - 5:00 PM; 30 minute lunch (unpaid)

JOB FUNCTIONS:

include, but not limited to, the following:

  • Completes patient and related documents in accordance with EHR//Meaningful Use standards and protocols and complies with PITU EHR policies including patient history, physical and vital information into the Tribe’s EHR system.
  • Maintains inventory and a maintenance schedule of supplies, ordering and restocking as needed to ensure availability for patient care. Prepares and submits orders for supplies and medications.
  • Assists with the cleaning of the facility.
  • Under the direction of the Primary Care Provider, assigns procedural and diagnostic coding in the Tribe’s EHR system.
  • Conducts pre-authorizations, referrals, etc. in accordance with third party insurer requirements.
  • Coordinates with the Business Office to assure billable services are being processed in a timely manner.
  • Performs complete intake process as specified by physician or other providers (chief complaint, history, vital signs, height, weight, OFC, care process model data, etc.). Prioritizes patient needs. Assists provider with examinations, procedures, treatments, and interventions.
  • Administers screening tests to patients (i.e., visual tests, audiology, spirometry, EKGs, skin-test etc.) within practice type.
  • Participates in the Vaccine for Children program by tracking immunizations, inventory control and reporting data as required.
  • Provides patient education and referrals as directed for services provided by the Behavioral Care Department.
  • Follow through with pharmacy refills, scheduling out of clinic procedures, obtaining, reporting, and tracking of lab results, leaving phone messages, and distributing faxes.
  • Coordination of Care:
    • Coordinates with Patient Support and Outreach team regarding referrals, support and follow up care.
    • Coordinates with care managers in other settings as appropriate.
    • Provides information on enabling services (e.g., transportation).
    • Maintains list of key community service agencies with contract information.
    • Provides information about recommended or available services and contract.
  • Support Patient in Self-Management and Behavior Change Using Motivational Interviewing and Coaching:
    • Assess readiness to change.
    • Assess and track patient capacity for and confidence in self-care.
    • Develops self-care plan in collaboration with patient.
    • Provides self-monitoring tools.
    • Provides or connects patients with support programs.
    • Assess and supports patient in adopting health behaviors; and assess and arranges treatment for mental health and substance abuse problems.
  • Team Base Care:
    • Works collaboratively with referring physician and other members of care team
    • Personalized Primary Care:
      • Completes pre-visit planning (reviews chart before visit, notifies patient of test needed before the visit).
      • Facilitates advanced care planning (Advanced Directives). Establishes process for reminder letters and phone calls.
      • Supports clinicians and team to achieve personalized primary care goals.
      • Facilitates transition of care (unscheduled hospital admissions, emergency department visits, skilled nursing home).
      • Ensures referrals follow the proper processes per the Referral Process and Purchased Referred Care policies and procedures.
      • Track status of referrals (i.e., surgeries).
      • Follow up and coordinate care with referral provider.
      • In collaboration with clinician, establish written care plan for patients transitioning from pediatrics to adult.
      • Provide information on health insurance resources.
      • Supervise and support health advocates.
      • Attend clinic team meetings and medical home meetings to assist with process design and help resolve team issues.
      • Support development of agenda for team meetings.
      • Review data summary on regular basis.
  • Attends training to stay current in field.
  • Maintains CLIA paperwork and standards.
  • Covers other staff when needed, including traveling to another clinic.
  • Cleans and sanitizes waiting room, bathroom, exam rooms, office spaces, etc. as needed.

General Requirements:

  • Must be insurable through the Tribe’s vehicle insurance required.
  • Must pass a criminal and fingerprint background check according to P.L. 101-630 standards required.
  • Must pass a pre-employment alcohol/drug screening required.
  • Must successfully complete the Medical Assistant credentialing process required.
  • Preference given to Spanish speaking applicants.

Education & Experience:

  • High School diploma, equivalent GED or higher required.
  • One (1) year experience working as a medical assistant required.
  • Valid Medical Assistant Certificate required.

License & Certification:

  • Must possess, maintain, and provide proof of a valid Driver’s License required.
  • Must obtain, maintain, and provide proof of a valid First Aid, CPR and/or BLS Certified (Red Cross or American Health Association) within three (3) months of hire required.
  • Must obtain, maintain, and provide proof of a valid Medical Assistant Certificate (National MA Certifications/Registration, Registered Medical Assistant of AMT, National Certified Medical Assistant, Certified Clinical Medical Assistant or Certified Medical Assistant of the AAMA) within six (6) months of hire required.
