Our Communities Connected for Kids Program provides services for children and families in the child welfare system, to ensure a community without abuse, neglect and abandonment. Based out of Port St. Lucie, FL, this position identifies post adoptive services to address the needs of substitute and adoptive families and serves as the agency liaison to all post adoptive caregivers.  Additional responsibilites of the position include:

  • Conduct one year follow-up contacts for post-adoptive families to identify the type of postadoption services requested or needed
  • Document outcome of post adoption contacts in FSFN and/or CCWIS.
  • Coordinate with community agencies and partners to promote the development of and access to adoption competent supports and services for adoptive families in Indian River, Martin, Okeechobee and Saint Lucie Counties
  • Respond to requests from CBC of Origin for post adoption services needs for families adopted elsewhere but residing in CBC service area
  • Participate in and provide follow up as needed on child specific staffings related to post adoption families/children
  • Gather information of the availability and effectiveness of post-adoptive services provided
  • Coordinate monthly adoption support groups and/or newsletter
  • Assist in resolving adoptive parent questions, complaints, grievances, and board payment issues.
  • Evaluate and recommend action on requests to DCF for enhanced maintenance adoption subsidy request
  • Participate in development and implementation of the Permanency section of the circuit’s five-year Prevention and Permanency Plan
  • Complete Extended Maintenance Adoption Subsidy (EMAS), notification, determination and termination
  • Complete Annual Cost of living when requested by adoptive parents
  • Complete the Tuition Waiver for all eligible youth attending college in Florida
  • Coordinate, manage documentation, and participate in Adoption Review Committee activities
  • Serve as Point of Contact for the Annual Employee State adoption benefits
  • Participate in the quality improvement process and accreditation process
  • Perform other duties as assigned.

Qualifications

EDUCATION:

Required:  Bachelor’s degree in human services, social work, or related field from an accredited school or university.

Preferred:  Master’s degree in human services, social work, or related field from an accredited school or university 

 

EXPERIENCE:

Required:  Three (3) or more years’ experience in adoption services and case management. Florida Certification Board Child Welfare case manager and or licensing certification.

Preferred:  Certification in adoption competence. FSFN and CCWIS experience preferred

 

Other Requirements:

  • Valid FL State Issued Driver's License
  • Work schedule hours will be a core Monday - Friday, dayshift; however flexibility to work outside of regular business hours is required including evenings and weekends due to the needs of the clients.

In the tradition of true partnership, Communities Connected for Kids works closely with several agencies that are funded through Medicaid, substance abuse/mental health or private funds to offer a continuum of treatment services otherwise unfunded by Community Based Care dollars. These partnerships help families strengthen their capacity to care for their children or help children heal from trauma they’ve experienced.
