Job Description

Clinical Pharmacist Specialists are responsible and accountable for the provision of safe, effective, and timely medication therapy. Through various assignments within the department, they support centralized and decentralized medication use systems while delivering optimal pharmacotherapy to patients within their specialty area. All pharmacists are responsible for medication preparation and distribution; evaluating, entering, and verifying medication orders; serving as a drug information resource for providers and other healthcare professionals; monitoring and managing medication regimens through protocols and consult services; and maintaining pharmacy inventory.

Job Duties

  • Provide safe, effective, and timely medication therapy for patients in the emergency department.

  • Deliver clinical pharmacy services through both centralized and decentralized medication use systems.

  • Provide both verbal and written consultation regarding medication therapy.

  • Provide medication-related education to patients, physicians, nurses, and other members of the healthcare team as requested.

  • Initiate, document, and participate in activities that promote positive patient outcomes, including medication use evaluations (MUEs), adverse drug event reporting, therapeutic drug monitoring, resolution of drug therapy questions, identification of drug interactions and allergies, clarification of complex medication orders, and review of non-formulary medication requests.

  • Review, verify, and enter medication orders for appropriateness, including drug selection, dosing, route, frequency, allergies, interactions, contraindications, therapeutic duplication, and compliance with institutional policies.

  • Monitor and manage medication regimens according to approved protocols, consults, and collaborative clinical practice standards.

  • Serve as the emergency medicine medication expert for high-acuity therapies, including procedural sedation agents, vasoactive medications, analgesics, neuromuscular blockers, thrombolytics, reversal agents, and other high-risk medications.

  • Respond to medical emergencies throughout the hospital, including Code Blues, stroke alerts, trauma activation, sepsis alerts, rapid response events, and other urgent clinical situations.

  • Assist with procedural sedations and rapid sequence intubations to ensure safe and accurate medication preparation and administration.

  • Support nursing staff in the safe use of programmable infusion pumps and other medication delivery technologies within the emergency department.

  • Ensure antimicrobial stewardship activities and opportunities are communicated with emergency department practitioners to optimize antimicrobial utilization, reduce inappropriate antibiotic use, and minimize antimicrobial resistance.

  • Participate in opioid stewardship initiatives, quality improvement activities, and medication-related process improvement efforts.

  • Support medication preparation, dispensing, distribution, and inventory management, including compliance with USP <795>, <797>, and <800> standards.

  • Participate in formulary management, medication policy development, automated dispensing cabinet optimization, and medication-use process improvement initiatives.

Required Education and Training

  • Graduate of an accredited College of Pharmacy; PharmD preferred.

  • Current, active Kentucky pharmacist license.

  • Minimum of three years of relevant job-related experience, or completion of a PGY-1 or PGY-2 ASHP-accredited pharmacy residency in the applicable area of practice in lieu of additional experience.

  • ACLS certification required within six months of hire.

  • Relevant (BCEMP, BCPS, or BCCCP) board certification required within one year of hire.

  • Demonstrated competency in emergency medicine pharmacotherapy, emergency response medication management, procedural sedation, and protocol-based clinical monitoring.

Preferred Education and Experience

  • Completion of a PGY-1 Pharmacy Residency.

  • Completion of a PGY-2 Emergency Medicine or Critical Care Pharmacy Residency preferred.

  • Prior experience in an emergency department or other high-acuity hospital practice setting.

  • Experience participating in multidisciplinary emergency department practice and hospital emergency response teams.

  • Advanced training or experience in emergency medicine pharmacotherapy, trauma care, toxicology, procedural sedation, thrombolytic therapy, high-risk medication management, and emergency response operations.
