Role Scope:Ā
The Physical Therapist Assistant (PTA) providesĀ high-qualityĀ rehabilitativeĀ support forĀ participantsĀ enrolled inĀ the HabitatĀ HealthĀ PACE program (Program ofĀ AllāInclusiveĀ Care for the Elderly). Working under the supervision of a licensed Physical Therapist, the PTA delivers therapeutic interventions that promote mobility, safety, independence, and overall quality of life. ThisĀ perādiemĀ role offers flexible scheduling while supporting participant needs across clinic,Ā home, andĀ dayācenterĀ settingsĀ in Sacramento.Ā
Key ResponsibilitiesĀ
- Therapeutic Interventions ā Implement individualized treatment plans including therapeutic exercise, gait training, balance activities, and functional mobility tasksĀ under the supervision of the PT.Ā
- Participant Assessment Support ā Observe, document, and report participant progress, functional changes, and safety concerns to the supervising PT.Ā
- Interdisciplinary Collaboration ā Coordinate with nurses, social workers, occupational therapists, and other PACE team members to ensure integrated,Ā personācenteredĀ care.Ā
- Home & Center-Based Care ā Provide therapy services in participant homes, theĀ PACE dayĀ center, and clinic environments as needed.Ā
- Equipment & Safety ā Train participants and caregivers on safe use of assistive devices and mobility equipment;Ā maintainĀ a safe therapeutic environment.Ā
- Documentation ā CompleteĀ accurate,Ā timelyĀ electronic health record (EHR) documentation in compliance with PACE, state, and federal guidelines.Ā
QualificationsĀ
- Accredited PTA Program graduate with an active California PTA license.Ā
- Current CPR/BLS certification.Ā
- Experience working with geriatric populations or inĀ longātermĀ care, home health, or PACE preferred.Ā
- Ability to work independently, travel locally, and adapt to changing participant needs.Ā
- Strong communication, documentation, and teamwork skills.Ā
Schedule & Work EnvironmentĀ
- Per diem / asāneededĀ typically 8am to 5pmĀ Ā
- Services delivered acrossĀ Habitat Health PACE Centers (Sacramento, South LA, San Leandro, West Covina)Ā participant homes.Ā
- Collaborative,Ā mission drivenĀ environment focused on supporting older adults to remainĀ safelyĀ in their homes.Ā
Compensation:Ā Ā
We take into account an individualās qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $40 - $46 per hour. The actual offer will be at the companyās sole discretion and determined by relevant business considerations, including the final candidateās qualifications, years of experience, skillset, and geographic location.Ā
